火针联合清肺健脾方治疗中重度痤疮的疗效及对血清IGF-1和DHEA水平的影响

摘　　要：目的:探讨火针联合清肺健脾方对中重度痤疮患者皮疹消退、瘢痕和血清胰岛素样生长因子-1(Insulin-like growth factor-1,IGF-1)、脱氢表雄酮(Dehydroepiandrosterone,DHEA)水平的影响。方法:选取笔者医院2021年1月-2022年1月收治的98例中重度痤疮患者作为研究对象,按照随机数字表法分为观察组(采用火针联合清肺健脾方治疗)49例与对照组(采用清肺健脾方治疗)49例,治疗时间2个月。观察两组治疗后皮损消退时间、血清IGF-1与DHEA水平、痤疮继发瘢痕权重评分(Echelle d'evaluation clinique des cicatrices d'acne,ECCA)、临床疗效、中医证候评分以及不良反应。结果:观察组囊肿消退、脓疱、丘疹、结节消退时间短于对照组(P<0.05)。治疗2个月后,观察组血清IGF-1、DHEA水平低于对照组(P<0.05)。观察组痤疮ECCA得分低于对照组(P<0.05)。观察组总有效率为97.96%,高于对照组85.71%(P<0.05)。观察组中医证候积分均低于对照组(P<0.05)。观察组不良反应发生率与对照组差异无统计学意义(P>0.05)。结论:火针联合清肺健脾方治疗中重度痤疮疗效确切,能够降低血清IGF-1、DHEA水平,改善瘢痕状况,安全性较高。Objective To explore the effect of fire-needle therapy combined with Qingfei Jianpi decoction on the skin rash subsidence,scar and levels of serum insulin-like growth factor-1(IGF-1)and dehydroepiandrosterone(DHEA)in patients with moderate-to-severe acne.Methods 98 patients with moderate-to-severe acne scars admitted to the hospital from January 2021 to January 2022 were selected as the research subjects,and were divided into observation group(49 cases,fire-needle therapy combined with Qingfei Jianpi decoction)and control group(49 cases,Qingfei Jianpi decoction)according to the random number table method,and both groups were treated for 2 months.The skin lesion subsidence time,serum IGF-1 and DHEA levels,echelle d'evaluation clinique des cicatrices d'acne(ECCA),clinical efficacy,TCM symptoms scores and adverse reactions were observed after treatment.Results The cyst subsidence time,pustule subsidence time,papula subsidence time and nodule subsidence time in observation group were shorter than those in control group(P<0.05).After 2 months of treatment,the levels of serum IGF-1 and DHEA were lower in observation group than those in control group(P<0.05).The acne ECCA scores in the observation group were lower than those in the control group(P<0.05).The total effective rate in observation group was 97.96%,which was higher than 85.71%in control group(P<0.05).The scores of TCM symptoms in observation group were lower compared to control group(P<0.05).The incidence rates of adverse reactions in observation group were not different from those in control group(P>0.05).Conclusion Fire-needle therapy combined with Qingfei Jianpi decoction has a definite efficacy,and it can reduce serum IGF-1 and DHEA levels and improve scar status,and it has high safety.
