摘　　要：目的:探究重组牛碱性成纤维细胞生长因子(rb-bFGF)凝胶联合强脉冲光(Intense pulsed light,IPL)治疗面部糖皮质激素依赖性皮炎(Facial corticosteroid addictive dermatitis,FCAD)的美学效果及对皮肤屏障功能的影响。方法:选取2020年1月-2021年12月笔者医院收治的FCAD患者,采用随机数字表法分为观察组和对照组,每组各30例。对照组采用IPL治疗,每4周治疗1次,共治疗4次;观察组在对照组的基础上联合rb-bFGF凝胶外涂治疗。比较两组患者的临床疗效,治疗前后症状评分、皮肤屏障功能指标水平变化,并进行复发率及安全性评价。结果:治疗后,两组症状评分均低于治疗前,且观察组主观症状、客观症状及皮损面积总积分均低于对照组(P<0.05)。观察组临床总有效率为93.33%,明显高于对照组的73.33%(P<0.05)。治疗后,观察组经皮水分丢失(Transepidermal water loss,TEWL)低于对照组,角质层含水量、表皮油脂含量高于对照组(P<0.05)。两组患者不良反应发生率、治疗后3个月内复发率比较,差异均无统计学意义(P>0.05)。结论:rb-bFGF凝胶联合IPL治疗FCAD临床疗效好,可明显改善患者面部红斑状况,减轻皮损,恢复皮肤屏障功能,且无明显不良反应。Objective To investigate the aesthetic effect of recombinant bovine basic fibroblast growth factor(rb-bFGF)gel combined with intense pulsed light(IPL)in the treatment of facial corticosteroid addictive dermatitis(FCAD)and the influence on skin barrier function.Methods Patients with FCAD who were admitted to the hospital from January 2020 to December 2021 were selected.They were divided into an observation group and a control group by random number table method with 30 cases in each group.The control group was treated with IPL,once every 4 weeks,for 4 times.On this basis,the observation group was treated with rb-bFGF gel.Clinical effects and changes in symptom scores and skin barrier function indicators after treatment were compared between groups.Recurrence and safety were evaluated.Results After treatment,symptom scores of both groups were lower than those before treatment.And the total scores for subjective symptoms,objective symptoms and lesion area in the observation group were lower than those in the control group(P<0.05).The total clinical effective rate in the observation group(93.33%)was significantly higher than that in the control group(73.33%)(P<0.05).After treatment,transepidermal water loss(TEWL)in the observation group was less than that in the control group.Water content in the stratum corneum and epidermal oil content were higher than those in the control group(P<0.05).There was no statistically significant difference between groups in terms of the incidence of adverse reactions and recurrence rate within 3 months after treatment(P>0.05).Conclusion rb-bFGF gel combined with IPL is effective in the treatment of FCAD,which can significantly improve facial erythema,reduce skin lesions and restore skin barrier function,without obvious adverse reactions.
