摘 要:目的基于MRI定量测量椎管内硬膜外脂肪含量与腰椎间盘突出的相关性。方法在T1WI矢状面上以各节段椎间盘水平为标准,测量L3~4、L4~5、L5~S1水平硬膜外脂肪(EF)前后径,椎管(SpiC)前后径,硬脊膜囊(DuS)前后径及腰椎间盘突出(LDH)的距离,并根据EF与SpiC测量值计算EF/SpiC。在T2WI矢状位图上视觉评估腰椎间盘Pfirrmann分级。结果LDH距离、EF前后径、DuS前后径、EF/Spic、Pfirrmann分级在性别之间差异均无统计学意义(P>0.05)。不同年龄段之间在LDH距离、EF前后径、DuS前后径的差异均无统计学意义(P>0.05),而在EF/Spic、Pfirrmann分级的差异有统计学意义(P<0.05)。L3~4椎间盘突出与DuS前后径相关(r=-0.612,P=0.000);L4~5椎间盘突出与DuS前后径相关(r=-0.514,P=0.000);而L5~S1椎间盘突出与EF前后径、EF/SpiC相关(r=-0.249,P=0.017;r=-0.297,P=0.004)。L3~4椎间盘Pfirrmann分级与DuS相关(r=-0.411,P=0.010),L4~5椎间盘Pfirrmann分级与DuS相关(r=-0.399,P=0.000)。结论L3~4、L4~5节段的椎间盘突出与相应EF含量无明显相关性,L5~S1节段EF的减少与LDH具有一定的相关性。Objective To quantitatively measure the correlation between intraspinal epidural fat content and lumbar disc herniation based on MRI.Methods The anterior and posterior diameters of epidural fat(EF),spinal canal(SpiC),spinal sac(DuS)and intervertebral disc herniation(LDH)were measured at L3-4,L4-s and Ls-S,levels in the sagital plane of T,WI.EF/SpiC was calculated according to the measured values of EF and SpiC.The Pfirrmann grade of lumbar intervertebral disc was evaluated visually on T,WI sagittal images.Results There were no significant differences in the distance of lumbar disc herniation,the anterior and posterior diameter of epidural fat,the anterior and posterior diameter of dural sac,EF/Spic and Pfirrmann grade between sexes(r=-0.411,P>0.05).There were no significant differences in the distance of lumbar disc herniation,the anterior and posterior diameter of epidural fat,and the anterior and posterior diameter of dural sac among different age groups(r=-0.411,P>0.05),but there were significant differences in EF/Spic and Pfirrmann grades(r=-0.411,P<0.05).L3-4 disc herniation was correlated with the anterior and posterior diameter of dural sac(r=-0.612,P=0.000).L4-s disc herniation was correlated with the anterior and posterior diameter of dural sac(r=-0.514,P=0.000).Ls-S,disc herniation was correlated with the anterior and posterior diameter of epidural fat and EF/SpiC(r=-0.249,P=0.017;R=-0.297,P=0.004).The Pfirrmann grade of L3-4 intervertebral disc was correlated with DuS(r=-0.411,P=0.010),and the Pfirrmann grade of L4-s intervertebral disc was correlated with Dus(r=-0.399,P=0.000).Conclusion There is no significant correlation between disc herniation in L,-4 and L4-s segments and the corresponding epidural fat content,while the decrease of epidural fat at Ls-S,segments has a certain correlation with the protrusion of intervertebral disc.
