The Molecular Genetic Basis of Differential Effects of Exercise Intervention on Noninfectious Chronic Disease from the Perspective of Precision Medicine

摘  要:精准医疗代表21世纪医学发展的方向,慢性非感染性疾病(noninfectious chronic disease,NCD)运动干预列入精准医疗范畴具有学理上的合理性。基于精准医学视角,对NCD患者运动干预效果差异的分子遗传学研究进展进行评述。结果显示:易感基因多态性在NCD发生、发展过程中具有重要作用;运动干预治疗主要依赖于运动刺激诱导释放的运动因子;运动因子通过一系列传导信号作用于NCD的分子靶点产生健康效益;NCD患者对运动干预治疗敏感性的个体差异取决于NCD相关基因的多态性,这些基因多态性不但决定了运动干预的有效性,同时也是NCD精准运动干预治疗的靶点。Precision medicine represents the direction of medical development in the 21st century,and it is theoretically reasonable to include noninfectious chronic disease(NCD)exercise intervention into the scope of precision medicine.The molecular genetic research progress on the differences of exercise intervention effects in patients with NCD was reviewed from the perspective of precision medicine.The results show that the genetic polymorphisms of susceptibility genes play an important role in the occurrence and development of NCD.Exercise intervention therapy mainly relies on the release of“exercise factors”induced by exercise stimulation.Exercise factors act on molecular targets of NCD through a series of signaling pathways to produce health benefits.The individual differences in sensitivity of NCD patients to exercise intervention therapy depend on the polymorphisms of NCD related genes,which not only determine the effectiveness of exercise intervention,but also serve as the targets of NCD precision exercise intervention therapy.
