基于脏腑学说探讨补肾通痹法对膝骨关节炎的疗效及炎性细胞因子的影响

摘　　要：目的:基于脏腑学说探讨补肾通痹法治疗膝骨关节炎(KOA)的疗效。方法:选取2019年3月至2020年4月就诊于石家庄市人民医院的KOA患者90例,按随机数字表法分为2组,每组45例,对照组给予服用双氯芬酸钠缓释胶囊,试验组给予补肾通痹汤联合艾灸及双氯芬酸钠缓释胶囊,治疗30 d,比较2组的膝关节疼痛视觉模拟量表评分(VAS)、西安大略麦克马斯特大学骨关节炎指数评分(WOMAC)、临床疗效及血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。结果:治疗后,试验组有效率高于对照组(P<0.05),2组VAS、WOMAC评分及血清IL-6、TNF-α水平比较,均较治疗前下降(P<0.05),且试验组低于对照组(P<0.05)。结论:补肾通痹法治疗KOA可有效减轻临床症状,降低炎性细胞因子水平,具有良好的临床疗效。Objective:to explore the therapeutic effect of Bushen Tongbi(tonifying the kidney and unblocking obstruction)method on knee osteoarthritis(KOA),based on the theory of viscera.Methods:Ninety KOA patients who visited Shijiazhuang People's Hospital from March 2019 to April 2020 were selected and randomly divided into two groups,with 45 cases in each group.The control group was given diclofenac sodium sustained-release capsules,while the experimental group was given Bushen Tongbi Tang combined with moxibustion and diclofenac sodium sustained-release capsules for 30 days to compare the visual analogue scale scores(VAS)of knee joint pain,Western Ontario McMaster University Osteoarthritis Index Score(WOMAC),clinical efficacy,and serum interleukin-6(IL-6)and tumor necrosis factorα(TNF-α).Results:After treatment,the effective rate of the experimental group was significantly higher than that of the control group(P<0.05),VAS,WOMAC scores and serum IL-6 and TNF-αlevels in both groups decreased(P<0.05),and the experimental group was lower than the control group(P<0.05).Conclusion:Bushen Tongbi method can effectively alleviate clinical symptoms and significantly reduce inflammatory cytokine levels in the treatment of KOA,with good clinical efficacy.
