儿童肝移植术后门静脉狭窄的超声特征及其诊断价值探讨

摘　　要：针对儿童肝移植术后出现的门静脉狭窄超声特征和诊断价值进行分析。选取2021年4月—2022年4月浙江大学医学院附属第一医院收治的100例儿童肝移植手术患儿作为研究对象,其中20例被超声诊断为门静脉狭窄,同时以门静脉CT血管重建联合门静脉数字减影血管造影结果为金标准,对其超声诊断儿童肝移植术后门静脉狭窄诊断效能和灵敏度、特异度和准确度进行分析。结果显示,接近金标准,两种检验方式在诊断结果上并未发现统计学差异(P>0.05)。研究发现,超声诊断多参数测量诊断门静脉狭窄敏感度和特异度相对较高,与金标准门静脉CT血管重建联合门静脉数字减影血管造影结果具有一致性,是儿童肝移植术后门静脉狭窄诊断的首选方案。Analyze the ultrasound characteristics and diagnostic value of portal vein stenosis after liver transplantation in children.100 pediatric liver transplant patients admitted to The First Affiliated Hospital Zhejiang University Medical College from April 2021 to April 2022 were selected as the research subjects.Among them,20 patients were diagnosed with portal vein stenosis by ultrasound.At the same time,the diagnostic efficacy,sensitivity,specificity,and accuracy of ultrasound in diagnosing portal vein stenosis in pediatric liver transplant patients were analyzed using the results of portal vein CT vascular reconstruction combined with portal vein digital subtraction angiography as the gold standard.The results showed that the specificity is 97.50%,and the accuracy is 97.00%,which is close to the gold standard.There was no statistical difference in the diagnostic results between the two test methods(P>0.05).Research has found that the sensitivity and specificity of multi parameter measurement in ultrasound diagnosis of portal vein stenosis are relatively high,which is consistent with the results of gold standard portal vein CT vascular reconstruction combined with portal vein digital subtraction angiography.It is the first choice for the diagnosis of portal vein stenosis in children undergoing liver transplantation.
