摘 要:目的了解社区卫生技术人员开展居家-社区医养结合服务意愿及其影响因素,为干预策略的拟定提供科学依据。方法采用分层抽样的方法,选取武汉市社区卫生服务机构781名卫生技术人员进行工作压力与薪酬、角色定位和开展居家-社区医养结合服务意愿等方面的问卷调查,采用有序Logistic回归分析法分析社区卫生技术人员开展居家-社区医养结合服务意愿的影响因素。结果工作压力处于较低水平和一般水平的社区卫生技术人员开展居家-社区医养结合服务意愿是工作压力较高水平者的3.44倍(P<0.01)和1.56倍(P<0.05);薪酬待遇与个人付出不匹配的社区卫生技术人员开展居家-社区医养结合服务意愿是匹配者的63.9%(P<0.01);角色定位为医养结合服务参与者和合作者的社区卫生技术人员开展居家-社区医养结合服务意愿是角色定位为主导者的1.7%(P<0.001)和22.0%(P<0.001)。结论工作压力大、薪酬待遇与个人付出不匹配和角色定位为医养结合服务参与者和合作者对社区卫生技术人员开展居家-社区医养结合服务意愿有显著的抑制作用。Objective To understand the willingness of community health technicians to provide integrated health and social care services of home-community and its influencing factors,so as to provide scientific basis for the formulation of intervention strategies.Methods Using hierarchical sampling,781 health technicians from Wuhan community health service institutions were selected to carry out the questionnaire survey on work pressure and salary,role positioning and willingness of integrated health and social care services of home-community.Orderly logistic regression analysis was used to analyze the factors influencing the willingness of community health technicians to provide integrated health and social care services of home-community.Results The willingness of community health technicians with low and general levels of work pressures to provide integrated health and social care services of home-community were 3.44 times(P<0.01)and 1.56 times(P<0.05)higher than those with high level work pressure.The willingness of community health technicians who do not match the salary and personal efforts was 63.9%(P<0.01)of the matching ones.The willingness of community health technicians whose roles defined as participants and collaborators of integrated health and social care services were 1.7%(P<0.001)and 22.0%(P<0.001)of those whose roles were defined as the leaders.Conclusion High work pressure,mismatch between salary and individual effort and role positioning of participants and collaborators have significant inhibitory effect on the willingness of community health technicians to provide integrated health and social care services of home-community.
