摘  要:目的了解深圳市居民医疗卫生服务利用和满意度的变化情况,为卫生行政部门制定区域卫生规划、合理配置卫生资源提供数据支持。方法采用多阶段分层整群随机抽样的方法,于2013年和2018年分别抽取深圳市居民3400人和3367人进行入户问卷调查。采用描述性统计分析方法进行比较。结果深圳市居民卫生服务可及性较好,2013年和2018年家庭与最近医疗机构距离2 km以内的分别占93.00%和90.17%,时间间隔15 min以内分别占86.33%和90.67%。65.31%的两周患病居民选择到医疗机构就诊,首诊选择社区卫生服务中心和市级医疗机构的比例增高;两次调查的住院率变化不显著,住院选择市级医院的比例由2013年的10.63%上升至2018年的24.36%。门诊直接医疗费用增幅(50.00%)与居民收入增幅(47.26%)接近,住院直接医疗费用增幅(60.00%)略高于居民收入增幅(47.26%)。相较2013年,2018年患病居民对门诊和住院的满意度均明显提高,分别提高了8.99%和9.82%。结论与2013年的调查结果相比,2018年深圳市居民的基本医疗卫生服务得到进一步保障,卫生服务的利用和就诊满意度有所提高。建议进一步加大对社区卫生服务机构和街道医院的支持,定期开展患者对医疗机构的满意度调查,不断提升卫生服务水平,进一步推动卫生改革与发展。Objective To investigate the changes with utilization and satisfaction of health service in Shenzhen City and to provide data support for health administration departments to formulate regional healthcare program and rationally allocate health resources.Methods The multi-stage stratified cluster random sampling method was applied to obtain study subjects.In 2013 and 2018,3400 and 3367 people were surveyed by questionnaire,respectively.Descriptive statistical analysis was used for comparison.Results The coverage of social medical insurance increased from 88.32%in 2013 to 95.31%in 2018.The distance and time interval between the family and the nearest medical institution were reduced.Residents had good accessibility to medical care.In 2013 and 2018,93.00%and 90.17%of families were within 2 kilometers of the nearest medical institution,respectively,and 85.58%and 90.33%were within 10 minutes of the time interval.65.31%of participants with illness in two weeks chose to go to medical institution for treatment.The proportion of community health service centers and municipal medical institutions selected for the first consultation had increased.There was no significant change in the hospitalization rate between the two-time surveys.The proportion of those who chose municipal hospitals increased from 10.63%in 2013 to 24.36%in 2018.The increase of direct outpatient medical costs(50.00%)was close to that of residents'income(47.26%),while the increase of direct hospitalization costs(60.00%)was slightly higher than that of residents'income(47.26%).Satisfaction of outpatient and inpatient was improved significantly by 8.99%and 9.82%,respectively.Conclusion Compared with the results in 2013,the basic medical and health services of Shenzhen residents in 2018 were further guaranteed,and the utilization of health services and the satisfaction of medical treatment were improved.It is suggested to further strengthen the support for community health service institutions and local hospitals,carry out regular surveys of patients'satisfa
