痰热清联合美洛西林治疗支气管肺炎患儿50例疗效和对CRP、IL-6、PCT水平及肺功能的影响

摘　　要：目的 探讨痰热清联合美洛西林治疗支气管肺炎患儿的效果及对CRP、IL-6、PCT水平、肺功能的影响。方法以随机数字表法将驻马店市第四人民医院2020年11月至2022年11月收治的100例支气管肺炎患儿分为联合组、参照组,各50例。参照组采用美洛西林治疗,联合组采用痰热清联合美洛西林治疗。比较两组临床疗效,治疗后症状消失时间,肺功能指标[呼吸总阻抗(Z5)、总气道阻力(R5)、中心气道阻力(R20)],病情相关指标[C反应蛋白(CRP)、血清白介素(IL-6)、降钙素原(PCT)],不良反应发生率。结果 联合组总有效率(96.00%)高于参照组(82.00%)(P<0.05);治疗后联合组症状(肺部啰音、发热、憋喘、气促、咳嗽)消退时间短于参照组(P<0.05);治疗后联合组肺功能指标(Z5、R5、R20)水平低于参照组(P<0.05);治疗后联合组CRP、IL-6、PCT水平低于参照组(P<0.05);联合组不良反应总发生率(6.00%)与参照组(10.00%)相比,差异无统计学意义(P>0.05)。结论 痰热清联合美洛西林治疗支气管肺炎患儿疗效确切,可有效改善机体炎症状态及肺功能,未增加明显不良反应。Objective To investigate the effect of tanreqing combined with mezlocillin in the treatment of children with bronchopneumonia and its influence on CRP,IL-6,PCT levels and pulmonary function.Methods 100 children with bronchopneumonia admitted to The Fourth People's Hospital of Zhumadian from November 2020 to November 2022 were divided into a combination group and a reference group using a random number table method,with 50 cases in each group.The reference group was treated with mezlocillin,and the combination group was treated with tanreqing combined with mezlocillin.Compare the clinical efficacy,symptom disappearance time after treatment,lung function indexes[total respiratory impedance(Z5),total airway resistance(R5),central airway resistance(R20)],condition-related indexes[C-reactive protein(CRP),serum interleukin(IL-6),procalcitonin(PCT)],and incidence rate of adverse reactions between the two groups.Results The total effective rate of the combination group(96.00%)was higher than that of the reference group(82.00%)(P<0.05);after treatment,the symptoms of the combination group(lung rales,fever,wheezing,shortness of breath,cough)subsided shorter than those of the control group(P<0.05);the levels of pulmonary function indexes(Z5,R5,R20)were lower in the combined group than in the reference group after treatment(P<0.05);after treatment,the levels of CRP,IL-6,and PCT in the combination group were lower than those in the reference group(P<0.05);there was no statistically significant difference in the total incidence of adverse reactions between the combination group(6.00%)and the reference group(10.00%)(P>0.05).Conclusion Tanreqing combined with mezlocillin is effective in the treatment of children with bronchopneumonia,which can effectively improve the inflammatory state and lung function without increasing obvious adverse reactions.
