变应性鼻炎患者TNF-α、Tim-1及TLR4变化与病情程度的相关性研究

摘　　要：目的探讨变应性鼻炎(AR)患者血清肿瘤坏死因子-α(TNF-α)、T细胞免疫球蛋白域黏蛋白域蛋白-1(Tim-1)、Toll样受体4(TLR4)变化与病情程度的相关性。方法选取南阳市第一人民医院2021年10月—2022年10月接受的77例AR患者为研究组,同期选取100例健康者为对照组,进行血清TNF-α、Tim-1、TLR4水平检测,并采用Pearson相关性分析法分析患者血清TNF-α、Tim-1、TLR4水平与病情程度关系;采用多因素Logistic回归分析影响AR的危险因素。结果相比对照组、研究组轻度与中度患者,重度患者血清TNF-α、Tim-1、TLR4水平及AR评分量表(SFAR)评分较高(P<0.05)。血清TNF-α、Tim-1、TLR4水平与SFAR评分呈正相关(P<0.05)。TNF-α、Tim-1、TLR4是影响AR病情程度的危险因素(P<0.05)。结论随着病情严重程度的发展,AR患者机体内血清TNF-α、Tim-1、TLR4水平升高,血清TNF-α、Tim-1、TLR4变化与SFAR评分呈正相关性,同时TNF-α、Tim-1、TLR4水平是影响AR严重程度的影响因素,临床可通过监测其水平变化对AR病情程度作出评定。Objective To investigate the correlations between the changes of tumor necrosis factorα(TNF-α),T cell immunoglobulin mucin-domain protein-1(Tim-1),toll-like receptor 4(TLR4)and the severity of the disease in patients with allergic rhinitis(AR).Methods A total of 77 AR patients received in the First People’s Hospital of Nanyang City from October 2021 to October 2022 were selected as the study group,and 100 healthy subjects were selected as the control group during the same period.Theri serum TNF-α,Tim-1 and TLR4 levels were detected.Pearson correlation analysis was used to analyze the relationships between serum TNF-α,Tim-1,TLR4 levels and the severity of the disease.Multivariate logistic regression was used to analyze the risk factors affecting AR.Results Compared with the control group and mild and moderate AR patients in the study group,serum levels of TNF-α,Tim-1,TLR4 and score for allergic rhinitis(SFAR)in severe AR patients were higher(P<0.05).Serum levels of TNF-α,Tim-1 and TLR4 were positively correlated with SFAR scores(P<0.05).TNF-α,Tim-1 and TLR4 were the risk factors affecting the severity of AR(P<0.05).Conclusions The serum levels of TNF-α,Tim-1 and TLR4 in AR patients are increased with the development of the severity of the disease,and the changes of serum TNF-α,Tim-1 and TLR4 are positively correlated with SFAR scores.Meanwhile,the TNF-α,Tim-1 and TLR4 are the influencing factors for the severity of AR,and ban be used to evaluate the degree of AR in clinic.
