摘　　要：标准化代谢性疾病管理中心(Metabolic Management Center, MMC)以“一个中心、一站服务、一个标准”为理念,推行糖尿病管理标准化、一站式解决方案。我院MMC建立之初,临床药师便受内分泌科邀请加入MMC诊疗团队,开设了MMC医药联合门诊。本文介绍我院临床药师参与MMC医药联合门诊,结合内分泌慢性病管理特点,为患者提供长程化药学服务的工作模式与实践。The Metabolic Management Center(MMC)takes“one center,one station service,one standard”as the concept,and promotes standardized and one-stop solutions for diabetes management.At the beginning of the establishment of MMC in our hospital,clinical pharmacists were invited by the Department of Endocrinology to join the MMC diagnosis and treatment team and a MMC medical joint outpatient clinic was opened.This article introduces the work mode and practice of clinical pharmacists participating in the MMC medical joint clinic outpatient service and providing patients with long-term pharmaceutical services by combining with the characteristics of endocrine chronic disease in our hospital.
