摘　　要：目的:通过系统评价与Meta分析方法研究结缔组织病相关间质性肺病(CTD-ILD)患者血清涎液化糖链抗原-6(KL-6)与肺功能、实验室指标的相关性。方法:系统检索知网、万方、维普、PubMed、Cochrane Library及Web of Science数据库,从建库至2021年9月18日关于CTD-ILD患者KL-6与肺功能、实验室指标相关性的研究,对符合纳入标准的文献采用R 4.02软件进行Meta分析。结果:共纳入25个研究,包含1846名CTD-ILD患者。Meta分析结果显示:肺功能指标与血清KL-6有不同程度的负相关性,相关系数-0.619~-0.272;乳酸脱氢酶与血清KL-6有相关性,相关系数为0.383,其余实验室指标与血清KL-6无相关性(P>0.05)或弱相关性(r=-0.195~0.091)。结论:血清KL-6能够反映出CTD-ILD患者肺功能与炎症的严重程度,可作为CTD-ILD患者早期病情判断与临床决策的参考指标。Objective:To investigate the correlation between Krebs von den lungen-6(KL-6)and lung function or laboratory indicators in connective tissue disease-associated interstitial lung diseases(CTD-ILD)by systematic review and meta-analysis.Methods:The literature about correlation between KL-6 and lung function or laboratory index in CTD-ILD patients were retrieved from CNKI,Wanfang,VIP,PubMed,Cochrane Library,and Web of Science databases from the establishment to September 18,2021.The metaanalysis was performed using R 4.02 software.Results:There were 25 studies enrolled,including 1846 CTDILD patients.The results of meta-analysis suggested that the indicators of lung function were negatively correlated with serum KL-6,with correlation coefficient from-0.619 to-0.272.Lactate dehydrogenase(LDH)was positively associated with serum KL-6,with the correlation coefficient of 0.383.There was no or weak correlation between other laboratory indexes and serum KL-6(P>0.05,or r=-0.195 to 0.091).Conclusion:Serum KL-6 can reflect the severity of pulmonary function and inflammation in patients with CTD-ILD,which can be used as a reference index for early diagnosis and clinical decision of CTD-ILD.
