摘  要:目的:探讨早产儿尿半胱氨酸白三烯E4(CysLTE4)和血清白介素-8(IL-8)水平变化与支气管肺发育不良的关系。方法:选取2021年1月至2022年12月蚌埠医学院第一附属医院新生儿重症监护病房(NICU)收治的胎龄<32周、出生体重<1500 g、且住院时间≥28天的58例早产儿作为研究对象。根据生后28 d是否发生BPD分为BPD组(n=21)和非BPD组(n=37),采集纳入病例出生第1、7、14、21、28天的尿液、血液标本,采用酶联免疫吸附法(ELISA)检测尿半胱氨酸白三烯E4(CysLTE4)、血清白介素-8(IL-8)水平。结果:生后第1天,BPD组尿CysLTE4、血清IL-8与非BPD组比较,差异无统计学意义(P>0.05)。随着日龄增长,两组尿CysLTE4和血清IL-8水平均呈现上升趋势,其中尿CysLTE4水平在第14天达高峰,血清IL-8水平在第21天达高峰,但同日BPD组尿CysLTE4和血清IL-8水平均高于非BPD组,差异有统计学意义(P<0.01)。结论:CysLTE4和IL-8与早产儿BPD的发生密切相关,动态监测早产儿尿CysLTE4和血清IL-8水平对BPD的早期诊断、早期干预及降低严重并发症的发生率、改善早产儿预后具有重要的临床意义。Objective:To investigate the relationship between urinary cysteinyl leukotriene E4(CysLTE4)and serum interleukin-8(IL-8)levels and bronchopulmonary dysplasia in premature infants.Methods:A total of 58 premature infants with a gestational age of<32 weeks,a birth weight of<1500 g and a hospital stay of≥28 days admitted to Neonatal Intensive Care Unit(NICU)of the First Affiliated Hospital of Bengbu Medical College from January 2021 to December 2022 were selected as the research objects.They were divided into BPD group(n=21)and non-BPD group(n=37)according to whether BPD occurred on day 28 after birth.Urine and blood samples from the included patients on days,1,7,14,21 and 28 after birth were collected.Urinary cysteinyl leukotriene E4(CysLTE4)and serum interleukin-8(IL-8)levels of two groups were detected by enzyme-linked immunosorbent assay(ELISA).Result:There was no significant difference in urinary CysLTE4 and serum IL-8 between BPD group and non-BPD group on day 1 after birth(P>0.05).Urinary CysLTE4 and serum IL-8 levels in both groups showed an upwards trend with the increase of age,and urinary CysLTE4 and serum IL-8 levels peaked on the 14th day and the 21st day,but urinary CysLTE4 and serum IL-8 levels in BPD group were higher than those in non-BPD group on the same day.The difference was statistically significant(P<0.01).Conclusion:CysLTE4 and IL-8 are closely related to the occurrence of BPD in preterm infants.Dynamic monitoring of urinary CysLTE4 and serum IL-8 levels in preterm infants has important clinical significance for early diagnosis and intervention of BPD,reducing the incidence of serious complications and improving the prognosis of preterm infants.
