雷珠单抗联合3D可视化手术系统下玻璃体切割治疗增生性糖尿病性视网膜病变效果探究

摘　　要：目的探究雷珠单抗联合3D可视化手术系统下玻璃体切割治疗增生性糖尿病性视网膜病变(prolifera-tive diabetic retinopathy,PDR)的临床疗效。方法回顾性分析2021年1月—2023年1月到福州东南眼科医院金山新院接受治疗的90例PDR患者,按照不同治疗方案分为对照组、研究组,各45例,对照组单独行3D可视化手术系统下玻璃体切割治疗,研究组在对照组治疗基础上联合术前雷珠单抗药物的注射。对比两组术中电凝止血次数、手术时间、术后视力改善情况及并发症发生率。结果与对照组相比,研究组术中电凝止血次数更低,手术时间更短,差异有统计学意义(P<0.05);两组患者术后矫正视力较术前均有改善,研究组患者术后矫正视力改善更加显著,差异有统计学意义(P<0.05);与对照组相比,研究组并发症发生率更低,差异有统计学意义(P<0.05)。结论PDR患者治疗中选择雷珠单抗联合3D可视化手术系统下玻璃体切割治疗方式,可减少患者术中电凝止血次数,缩短手术时间,改善患者术后视力,降低并发症发生率。Objective To investigate the clinical efficacy of proliferative diabetic retinopathy(PDR)treated with vit-rectomy under Lucentis combined with 3D visualization surgical system.Methods A retrospective analysis was per-formed on 90 patients with PDR who received treatment in Jinshan New Hospital,Southeast Eye Hospital,Fuzhou from January 2021 to January 2023,and they were divided into control group and study group according to different treatment plans,with 45 cases in each group.The control group received vitrectomy under 3D visual surgical system alone,and the research group combined with preoperative injection of leizumab on the basis of the treatment of the control group.The frequency of hemostasis,operation time,postoperative visual improvement and complication rate were compared between the two groups.Results Compared with the control group,the number of intraoperative elec-trocoagulation was lower than that in the study group and the operation time was shorter,and the difference was statis-tically significant(P<0.05).The postoperative corrected visual acuity in both groups was improved compared with that before surgery,and the postoperative corrected visual acuity in the study group was more significantly improved,and the difference was statistically significant(P<0.05).Compared with the control group,the complication rate of the study group was lower,and the difference was statistically significant(P<0.05).Conclusion The choice of Lucentis combined with 3D visualization surgical system under vitrectomy treatment modality in the treatment of PDR patients can reduce the number of intraoperative electrocoagulation and hemostasis of patients,shorten the operation time,im-prove the patients'postoperative visual acuity,and reduce the rate of complications.
