健脾化湿法联合宫颈环形电切术治疗宫颈上皮内瘤变的疗效及其对免疫功能的影响

摘　　要：目的探究健脾化湿法联合宫颈环形电切术(LEEP)治疗宫颈上皮内瘤变(CIN)的疗效及其对免疫功能的影响。方法选取2021年1月至2022年1月临沂市中心医院收治的80例CIN患者作为研究对象,利用随机数字表法分为对照组(n=40,采用LEEP治疗)与研究组(n=40,采用健脾化湿法联合LEEP治疗),收集患者临床资料,并比较两组的疗效、免疫细胞及炎症指标的变化、并发症的发生率。结果与对照组相比,研究组总有效率显著增加(P<0.05)。研究组分泌物过多、腰骶酸痛及血性白带的消失时间均显著短于对照组(P<0.05)。治疗后,两组CD4+、CD4+/CD8+及白介素(IL)-10水平均显著升高(P<0.05),CD8+、IL-6及肿瘤坏死因子(TNF)-α水平均显著降低(P<0.05),且研究组变化更为显著(P<0.05)。治疗3个月后,研究组人乳头瘤病毒(HPV)转阴率显著高于对照组(P<0.05)。与对照组相比,研究组并发症总发生率显著降低(P<0.05)。结论健脾化湿法联合LEEP治疗CIN效果显著,可显著改善患者的临床症状,抑制炎症因子的表达,增强患者的免疫功能,并减少并发症的发生。Objective To explore the therapeutic effect of Jianpi Huashi therapy combined with cervical loop electrosurgical excision procedure(LEEP)in the treatment of cervical intraepithelial neoplasia(CIN)and its influence on immune function.Methods A total of 80 patients with CIN admitted to Linyi Central Hospital from January 2021 to January 2022 were selected as research subjects and randomly divided into the control group(n=40,treated with LEEP)and the study group(n=40,treated with Jianpi Huashi therapy combined with LEEP)by random number table method.The clinical data of patients were collected,and the curative effect,changes of immune cells,inflammatory indicators and the incidence of complications were compared between the two groups.Results Compared with the control group,the total effective rate of the study group was obviously increased(P<0.05).The disappearance time of excessive secretion,lumbosacral pain and bloody leucorrhea in the study group was obviously lower than that in the control group(P<0.05).After treatment,the levels of CD4+,CD4+/CD8+and interleukin(IL)-10 in CIN patients in both groups increased obviously(P<0.05),the levels of CD8+,IL-6 and tumor necrosis factor(TNF)-αdecreased obviously(P<0.05),the changes in the study group were more significant(P<0.05).After 3 months of treatment,the negative conversion rate of human papilloma virus(HPV)in the study group was significantly higher than that in the control group(P<0.05).Compared with the control group,the total incidence of complications in the study group was obviously lower(P<0.05).Conclusions The combination of Jianpi Huashi therapy and LEEP is effective in treating CIN,which can obviously improve the clinical symptoms of patients,inhibit the expression of inflammatory factors,enhance the immune function of patients,and reduce the incidence of complications.
