摘 要:目的 探讨上皮钙黏素在年轻女性乳腺癌中的表达及临床意义。方法 回顾性分析河南科技大学第一附属医院2016年1月至2019年9月收治的74例年轻女性乳腺癌患者临床资料。运用免疫组化检测患者上皮钙黏素的表达情况,分析其与临床病理特征的相关性,分析患者生存及预后危险因素。结果 患者上皮钙黏素表达在淋巴结转移、肿瘤大小、TNM分期、组织学分级、雌激素受体、孕激素受体、三阴型与Ki-67表达有关(P<0.05),与HER-2表达无关(P>0.05)。上皮钙黏素低表达患者的无病生存率和总生存率低于高表达患者(P<0.05)。Cox回归单因素分析显示淋巴结转移、肿瘤大小、TNM分期和上皮钙黏素表达是影响年轻女性乳腺癌患者无病生存率的因素(P<0.05)。Cox比例风险模型进行多因素分析,结果显示TNM分期是影响年轻女性乳腺癌患者无病生存率和总生存率的因素(P<0.05)。结论 上皮钙黏素低表达的年轻女性乳腺癌患者预后更差,上皮钙黏素可能成为年轻女性乳腺癌患者的诊断、治疗及评估预后的靶点。Objective To investigate the expression and clinical significance of E-cadherin in young women with breast cancer.Methods The clinical data of 74 young female breast cancer patients admitted to the First Affiliated Hospital of Henan University of Science and Technology from January 2016 to September 2019 were retrospectively analyzed.Immunohistochemistry was used to detect the expression of E-cadherin in patients,analyze its correlation with clinicopathological characteristics,and analyze the survival and prognostic risk factors of patients.Results The expression of E-cadherin in patients with lymph node metastasis,tumor size,TNM stage,histological grade,estrogen receptor,progesterone receptor,three negative type was related to Ki-67 expression(P<0.05),but not related to HER-2 expression(P>0.05).The disease free survival rate and overall survival rate of patients with low expression of E-cadherin were lower than those with high expression(P<0.05).Cox regression univariate analysis showed that lymph node metastasis,tumor size,TNM stage and E-cadherin expression were the factors influencing disease-free survival rate of young women with breast cancer(P<0.05).Cox proportional hazards model was used for multivariate analysis.The results showed that TNM stage was a factor affecting disease-free survival rate and overall survival rate of young women with breast cancer(P<0.05).Conclusion Young women with low expression of E-cadherin have a worse prognosis in breast cancer.E-cadherin may become a target for diagnosis,treatment and prognosis evaluation of young women with breast cancer.
