摘  要:目的 探讨甲状腺功能亢进患者骨密度、骨代谢、脂肪细胞因子的相关性。方法 纳入2021年12月至2022年12月郑州市第七人民医院收治的87例甲状腺功能亢进患者为研究对象,根据骨密度T值分为骨量正常组、骨量减少组和骨质疏松组,检测各组患者脂肪细胞因子(内脂素、瘦素、脂联素、抵抗素)和骨代谢指标[骨源性碱性磷酸酶(BALP)、骨钙素(OCN)、25-羟基维生素D(25-OH-VD)、Ⅰ型胶原交联C末端肽(CTXⅠ)]水平,分析脂肪细胞因子与甲状腺功能亢进患者骨代谢和骨密度的相关性。结果 经骨密度检查,87例甲亢患者中骨量正常23例,骨量减少35例,骨质疏松29例。3组性别、年龄、体重指数、病程、疾病类型比较,差异无统计学意义(P>0.05)。骨质疏松组、骨量减少组、骨量正常组OCN、BALP、CTXⅠ、瘦素、脂联素、抵抗素水平依次降低,25-OH-VD水平、内脂素依次升高,差异有统计学意义(P<0.05)。经Pearson相关性分析,内脂素与OCN、BALP、CTXⅠ呈负相关(r<0,P<0.05),与25-OH-VD呈正相关(r>0,P<0.05);瘦素、脂联素及抵抗素与OCN、BALP、CTXⅠ呈正相关(r>0,P<0.05),与25-OH-VD呈负相关(r<0,P<0.05)。结论 甲状腺功能亢进患者骨密度与骨代谢、脂肪细胞因子相关,临床可根据患者脂肪细胞因子水平和骨代谢水平采取治疗措施,预防骨质疏松的发生。Objective To explore the correlation between bone mineral density,bone metabolism and adipocytokines in patients with hyperthyroidism.Methods Eighty-seven patients with hyperthyroidism admitted to the 7th People’s Hospital of Zhengzhou from December 2021 to December 2022 were included as the research objects.Patients were divided into normal bone mass group,reduced bone mass group,and osteoporosis group based on the bone mineral density T-value.The levels of adipose cytokine(visfatin,leptin,adiponectin,resistin)and bone metabolism indexes[bone alkaline phosphatase(BALP),osteocalcin(OCN),25-hydroxyvitamin D(25-OH-VD),typeⅠcollagen cross-linked C-terminal peptide(CTXⅠ)]were detected in each group.The correlation between adipocytokines and bone metabolism and bone mineral density in patients with hyperthyroidism was analyzed.Results After bone mineral density examination,among 87 patients with hyperthyroidism,23 cases had normal bone mass,35 cases had decreased bone mass and 29 cases had osteoporosis.There was no statistical differences in gender,age,body mass index,course of disease and disease type among the three groups(P>0.05).The levels of OCN,BALP,CTXⅠ,leptin,adiponectin and resistin in osteoporosis group,osteopenia group and normal osteopenia group decreased in turn,while the levels of 25-OH-VD and visfatin increased in turn,with statistical significance(P<0.05).Pearson correlation analysis showed that visfatin was negatively correlated with OCN,BALP and CTXⅠ(r<0,P<0.05)and positively correlated with 25-OH-VD(r>0,P<0.05),leptin,adiponectin and resistin were positively correlated with OCN,BALP and CTXⅠ(r>0,P<0.05)and negatively correlated with 25-OH-VD(r<0,P<0.05).Conclusion Bone mineral density of patients with hyperthyroidism is related to bone metabolism and adipocytokines,and clinical treatment measures can be taken according to the level of adipocytokines and bone metabolism to prevent osteoporosis.
