摘 要:目的 分析骨形态发生蛋白4(BMP4)与2型糖尿病患者胰岛素抵抗的关系。方法 本研究为回顾性分析,收集医院2021年10月至2022年10月收治的100例2型糖尿病患者临床资料,根据是否合并胰岛素抵抗将患者分为胰岛素抵抗组(85例)和非胰岛素抵抗组(15例),所有患者均完成BMP4水平检测。详细统计2型糖尿病患者临床资料,比较两组临床资料及BMP4水平,分析BMP4与胰岛素抵抗发生的关系。结果 非胰岛素抵抗组与胰岛素抵抗组吸烟史、体重指数、BMP4水平比较,差异有统计学意义(P<0.05);两组性别、年龄、病程、饮酒史、糖尿病家族史、收缩压、舒张压比较,差异无统计学意义(P>0.05);两组BMP4水平、胰岛素抵抗指数(HOMA-IR)比较,差异有统计学意义(P<0.05);经Pearson相关性分析,2型糖尿病患者BMP4水平与HOMA-IR呈正相关(r=0.250,P=0.012);logistic回归分析结果显示,BMP4水平高、有吸烟史、体重指数高是2型糖尿病患者胰岛素抵抗的危险因素(OR>1,P<0.05)。结论 BMP4水平、吸烟史及体重指数与2型糖尿病患者胰岛素抵抗有密切关系,BMP4水平越高,发生胰岛素抵抗风险越大。Objective To analyze the relationship between bone morphogenetic protein 4(BMP4)and insulin resistance in type 2 diabetes patients.Methods The clinical data of 100 patients with type 2 diabetes admitted to the hospital from October 2021 to October 2022 were collected for retrospective analysis.The patients were divided into insulin resistance group(85 cases)and non-insulin resistance group(15 cases)according to whether they were combined with insulin resistance.All patients were tested for BMP4 level.The clinical data of type 2 diabetes patients were collected in detail,and the clinical data and BMP4 levels of the two groups were compared to analyze the relationship between BMP4 and insulin resistance.Results There were statistically significant differences in smoking history,body mass index and BMP4 level between non-insulin resistance group and insulin resistance group(P<0.05).There was no statistically significant difference in gender,age,course of disease,drinking history,family history of diabetes,systolic blood pressure and diastolic blood pressure between the two groups(P>0.05).There was statistically significant difference in BMP4 level and HOMA insulin resistance index(HOMA-IR)index between the two groups(P<0.05).Pearson correlation analysis showed that the level of BMP4 in type 2 diabetes patients was positively correlated with HOMA-IR(r=0.250,P=0.012).Logistic regression analysis showed that high BMP4 level,smoking history and high body mass index were risk factors for insulin resistance in type 2 diabetes patients(OR>1,P<0.05).Conclusion BMP4 level,smoking history and body mass index are closely related to insulin resistance in type 2 diabetes patients,the higher the BMP4 level,the greater the risk of insulin resistance.
