摘　　要：目的:分析三种炎性标志物C反应蛋白(C-reactive protein,CRP)、乳酸脱氢酶(lactate dehydrogenase,LDH)和降钙素原(procalcitonin,PCT)在细菌性肺炎与病毒性肺炎患者中的诊断效能。方法:选取2022年1月至2023年4月苏州大学附属第一医院就诊的患者230例,其中细菌性肺炎患者127例,病毒性肺炎患者103例。收集细菌性肺炎与病毒性肺炎患者入院后CRP、LDH及PCT的检测结果,经KruskalWallis秩和检验筛选数据后再通过绘制受试者工作特征(receiver operating characteristic,ROC)曲线对筛选出的数据进行分析,判断各个指标的诊断效能。结果:通过ROC曲线分析CRP、LDH和PCT对两类肺炎的诊断效能,发现PCT的指标P<0.05,ROC曲线成立。经计算得出曲线下面积0.778,诊断临界点0.40ng/mL处的敏感度为0.69,特异度为0.82(P<0.01)。与LDH和CRP相比,PCT对两类肺炎的诊断效能较高。结论:PCT可作为鉴别细菌性肺炎与病毒性肺炎的优选炎症指标,具有一定的诊断价值。Objective:To analyze the efficacy of three inflammation indicators C-reactive protein(CRP),lactate dehydrogenase(LDH)and procalcitonin(PCT)in the diagnosis of bacterial pneumonia and viral Bacterial pneumonia.Method:The total 230 cases including 127 bacterial pneumonia and 103 viral pneumonia patients were collected at the First Affiliated Hospital of Soochow University from Jan.2022 to Apr.2023.The CRP,LDH and PCT data of these patients were detected and analyzed via Receiver Operating Characteristic(ROC)according to KruskalWallis test result.Result:The diagnosability of these three indictors on both pneumonias indicated that PCT was found to be P<0.05,and the ROC curve was established.The area under the curve was calculated to be 0.778.The sensitivity at the diagnostic critical point of 0.40 ng/mL was 0.69,with the specificity 0.82,and(P<0.01).Compared with LDH and CRP,PCT has the best performance for diagnosing these two types of pneumonia.Conclusion:PCT can be used as a preferred inflammatory indicator to differentiate bacterial pneumonia and viral pneumonia,and has certain diagnostic value.
