乳腺癌患者及其配偶复发恐惧的潜在剖面分析

摘　　要：目的了解乳腺癌患者及配偶复发恐惧的潜在类别,并探讨其影响因素。方法采用患者恐惧疾病进展简化量表与配偶恐惧疾病进展简化量表对2022年5-11月于潍坊医学附属医院甲乳外科接受治疗的273例乳腺癌患者及其配偶进行调查,并对乳腺癌患者夫妻的复发恐惧进行潜在剖面分析(LPA),采用无序多分类logistic回归分析潜在类别的影响因素。结果乳腺癌患者及配偶存在3种复发恐惧潜在类别,分别是“患者-配偶共低组”(C1,31.4%)、“患者高-配偶低组”(C2,36.6%)和“患者-配偶共高组”(C3,32.0%)。无序多分类logistic回归分析表明复发恐惧的不同类别受到年龄、确诊时长、家庭收入、配偶受教育程度、益处发现的影响。结论乳腺癌患者及配偶复发恐惧表现存在异质性,应制定与实施二元视角下以乳腺癌夫妻为核心的临床护理心理干预。Objective To understand the potential categories of fear of recurrence in patients with breast cancer and their spouses,and to explore their influencing factors.Methods A total of 273 breast cancer patients and their spouses who were treated in the depratment of breast surgery of a tertiary hospital in Shandong province from May to November 2022 were surveyed by using the Fear of Progression Questionnaire Short Form(FoP-Q-SF)for Patients and Fear of Progression Questionnaire for Partners(FoP-Q-SF/P)for Spouses,and the lafent profile analysis(LPA)was used to categorize the recurrence fear of breast cancer couples,and the influencing factors of potential categories was analyzed by unordered multicategorical logistic regression.Results There were 3 potential categories of recurrence fear among breast cancer patients and their spouses,namely"patient-spouse co-low group"(C1,31.4%),"patient-high-spouse-low group"(C2,36.6%),and"patient-spouse co-high group"(C3,32.0%).Unordered multicategorical logistic regression analysis showed that different subgroups were influenced by couple's age,length of diagnosis,household income,spouse's education,and benefit finding(P<0.05).Conclusion There is heterogeneity in the expression of recurrence fear in breast cancer patients and their spouses,and a couple-focused clinical nursing psychological interventions from a dyadic perspective shall be developed and implemented.
