基于数据挖掘探讨袁全才治疗失眠的用药规律

摘　　要：目的:基于数据挖掘技术分析袁全才主任医师治疗失眠的用药规律。方法:收集2022年1月1日至2022年12月31日袁师治疗失眠的病案,将筛选后的中药处方录入Excel表格建立数据库,采用SPSS Modeler 18.0对药物进行关联分析,SPSS 20.0对药物进行频数分析、聚类分析及相关性分析,采用Ucinet 6.0对高频药物构建核心网络图。结果:共收集163则病案,排前3位的证型为肝郁脾虚证、邪郁少阳证、痰浊中阻证;涉及药物97味,总频次为2110次,其中高频药物36味,前5位分别为甘草、半夏、柴胡、远志、黄芩。排前3位的功效依次为补虚、安神、解表。药性以平、温、寒为主,药味以甘、辛、苦为主,归经以肺、肝、脾经为主。关联规则分析获得药对46对、药物组合21组,聚类分析得出潜在药物组合9类,并根据Pearson相关性分析获得核心组方5组。结论:袁师治疗失眠以和解少阳、调畅气机、清热除烦等为主要治法,常用小柴胡汤、柴胡桂枝干姜加龙骨牡蛎汤、温胆汤合菖蒲郁金汤、酸枣仁汤合当归芍药散、僵蚕二黄散等经典方治疗。Objective:To investigate the medication rule of Yuan Quancai in the treatment of insomnia based on data mining.Methods:Medical records were collected from the patients with insomnia who were treated by Yuan Quancai from January 1st to December 31st,2022,and after screening,traditional Chinese medicine prescriptions were entered into Excel to establish a database.SPSS Modeler 18.0 was used to perform the association rule analysis;SPSS 20.0 was used to perform frequency analysis,cluster analysis,and correlation analysis for drugs;Ucinet 6.0 was used to establish a core network for high-frequency drugs.Results:A total of 163 medical records were collected,and the top 3 syndrome types were liver depression and spleen deficiency,stagnant pathogen of Shaoyang,and phlegm retention obstruction in the middle-Jiao.A total of 97 drugs were involved,with a total frequency of 2110 times,among which there were 36 high-frequency drugs,with the top 5 drugs of Radix Glycyrrhizae,Pinellia ternata,Bupleurum chinense,Polygala tenuifolia,and Scutellaria baicalensis.The top 3 functions were deficiency-tonifying herbs,tranquilizing drugs,and exterior syndrome-relieving drugs.Most drugs were neutral,warm or cold in nature and had a sweet,pungent or bitter taste,and the drugs mainly entered the lung,liver,and spleen meridians.The association rule analysis obtained 46 drug pairs and 21 drug combinations,the cluster analysis obtained 9 categories of potential drug combination,and the Pearson correlation analysis obtained 5 core prescriptions.Conclusion:Yuan Quancai mainly uses the treatment methods of harmonizing Shaoyang,activating and harmonizing Qi-movement,and clearing heat and relieving fidgetiness in the treatment of insomnia,as well as the classic prescriptions of Xiaochaihu decoction,Longmu Muli decoction with Bupleurum chinense,Ramulus Cinnamomi,and dried ginger,Wendan decoction combined with Changpu Yujin decoction,Suanzaoren decoction combined with Danggui Shaoyao powder,and Jiangcan Erhuang powder.
