摘  要:目的分析孕妇孕期甲状腺功能减退和铁的关系,为临床预防孕妇甲状腺功能减退症提供依据。方法选取2021年8月—2022年2月在甘肃省妇幼保健院门诊产检的孕妇356例,以问卷调查的方式获取孕妇的一般社会人口学资料,通过医院病案查询系统查询孕妇孕早期血红蛋白(Hb)、总蛋白(TP)、白蛋白(ALB)、球蛋白(GLO)、促甲状腺激素(TSH)、血清游离T4(FT4)、抗甲状腺过氧化物酶抗体(TPOAb)、血清铁蛋白(SF)等资料,用SPSS25.0进行统计分析。结果按照省妇幼保健院制定的特异性临界值参考范围(9.8 pmol/L<T4<22 pmol/L、TSH>4.0 mU/L)分为病例组124例和健康对照组232例。两组Hb、TSH、FT4、SF水平差异均有统计学意义(P<0.01)。Hb水平和FT4水平呈正相关,和SF水平呈正相关,和TSH水平呈负相关(r=0.26,r=0.226,r=-0.398,P<0.01);SF水平和FT4水平呈正相关,和TSH水平呈负相关(r=0.136,r=-0.134,P<0.05)。结论孕早期孕妇甲状腺功能受血清总蛋白水平的影响,及时补充铁剂可以预防和纠正亚临床甲状腺功能减退。Objective To analyze the relationship between iron and hypothyroidism during early pregnancy,provide an evidence for clinical prevention of hypothyroidism in pregnant women.Methods A total of 356 pregnant women receiving prenatal examination in Department of Outpatient in Gansu Maternal and Child Health Care Hospital were selected from August 2021 to February 2022.The general social and demographic data of the pregnant women were obtained by questionnaire survey.The data of hemoglobin(Hb),total protein(TP),albumin(ALB),globulin(GLO),thyroid stimulating hormone(TSH),free thyroxine(FT4),thyroid peroxidase antibody(TPOAb),and serum ferritin(SF)were obtained by hospital medical record query system.The results were analyzed statistically with SPSS 25.0 software.Results According to the critical value(9.8 pmol/L<T4<22 pmol/L,TSH>4.0 mU/L),the pregnant women were divided into case group(124 cases)and healthy control group(232 cases).There were statistically significant differences in Hb,TSH,FT4,and SF levels between the two groups(P<0.01).Hb level was positively correlated with FT4 level and SF level,Hb level was negatively correlated with TSH level(r=0.26,r=0.226,r=-0.398,P<0.01).SF level was positively correlated with FT4 level and negatively correlated with TSH level(r=0.136,r=-0.134,P<0.05).Conclusion Thyroid function of pregnant women in early pregnancy is affected by serum total protein level.Timely iron supplementation can prevent and correct subclinical hypothyroidism.
