摘 要:目的:探讨血浆致动脉硬化指数(AIP)对冠心病(CHD)合并2型糖尿病(T2DM)病人冠状动脉病变程度的预测价值。方法:选取2018年1月1日—2020年12月1日中日友好医院中西医结合心脏内科首次确诊CHD或初次行支架置入的病人208例,其中CHD组95例,CHD合并T2DM组113例。收集病人一般资料、生化指标及冠状动脉造影结果,分析两组之间的差异及不同冠状动脉病变支数组间的差异。结果:CHD合并T2DM组AIP高于CHD组(P=0.044)。将CHD合并T2DM病人根据冠状动脉病变支数分为单支病变组、双支病变组、三支病变组。3组AIP、TG比较,差异均有统计学意义(P<0.05)。多元Logistic回归模型显示,AIP、CHD家族史是影响冠状动脉病变支数的独立危险因素。受试者工作特征(ROC)曲线表明,AIP可较好地预测冠状动脉病变支数[ROC曲线下面积(AUC)=0.684,P=0.009]。结论:AIP是CHD合并T2DM的危险因素,也是CHD合并T2DM病人评估冠状动脉病变支数的独立危险因素,并作为预测冠状动脉病变程度的可行性指标。Objective:To observe the predictive value of atherogenic index of plasma(AIP)for the degree of coronary artery disease in patients with coronary heart disease(CHD)combined with type 2 diabetes mellitus(T2DM).Methods:Two hundred and eight patients with from January 1,2018 to December 1,2020 were selected,there were 95 cases in the CHD group and 113 cases in the CHD combined with T2DM group.The general data,biochemical indexes,and coronary angiography results were collected,and compared between two groups.The number of coronary artery lesion branches in different coronary arteries were analyzed.Results:AIP was higher in the CHD combined T2DM group than that in the CHD group(P=0.044).According to the number of coronary artery lesion branches,patients with CHD combined with T2DM were divided into single vessel disease group,double vessel disease group,and triple vessel disease group.The AIP and triacylglycerol(TG)between three groups were compared,and the difference was statistically significant(P<0.05).The multivariate Logistic regression model showed that AIP was an independent risk factor affecting the number of coronary artery lesion branches(OR=15.218).The receiver operating characteristic(ROC)curve showed that AIP was a better predictor of the number of coronary artery lesion branches[ROC area under the curve(AUC)=0.684,P=0.009].Conclusion:AIP was a risk factor for CHD combined with T2DM,and it was an independent risk factor for evaluating the number of coronary artery lesion branches in patients with CHD combined with T2DM.It could be used as a feasible index to predict the degree of coronary artery disease.
