摘　　要：健康管理模式是一种以人为中心,通过对个体或群体在社会环境、生物因素及心理因素等方面进行干预和控制,以实现减少疾病发生、促进健康为目的的新模式。对慢性病患者进行健康管理,实行全过程动态监控,对慢性病防治起到积极的效果,本文就健康管理应用于慢性病防治工作中的问题及解决方法展开综述。The health management model is a new model that is people-centered and aims to reduce the occurrence of diseases and promote health by intervening and controlling the social environment,biological factors and psycho⁃logical factors of individuals or groups.The health management of patients with chronic diseases and the implementa⁃tion of dynamic monitoring of the whole process have a positive effect on the prevention and treatment of chronic dis⁃eases.This article reviews the problems and solutions of health management in the prevention and treatment of chronic diseases.
