蒙医温针辅助治疗腮腺术后面神经损伤的临床研究

摘　　要：选取2021年1月至2022年6月的口腔颌面外科发生腮腺术后面神经损伤患者60例,随机平均分为研究组和对照组,研究组采用蒙医温针治疗,对照组采用常规治疗,对比两种治疗方法的临床疗效、House⁃Brackmann面神经功能分级、神经肌电图(compound muscle action potential,CMAP)指标、患者心理状况及生活质量(the mositem short from health survey,SF⁃36量表)。结果表明:(1)研究组患者总有效率显著高于对照组(P<0.05);(2)治疗后研究组及对照组House⁃Brackmann量表等级明显升高,以研究组处于Ⅰ级、Ⅱ级患者居多,两组差异显著(P<0.05);(3)治疗后两组CMAP较治疗前增高,BR较治疗前缩短,差异显著(P<0.05);(4)治疗后,两组患者心理评分较前升高,研究组显著高于对照组(P<0.05);(5)治疗后,两组患者SF⁃36量表评分较前升高,研究组显著高于对照组(P<0.05)。蒙医温针治疗可明显改善腮腺术后面神经损伤患者的House⁃Brackmann面神经功能分级、神经肌电图指标,促进患者面神经功能恢复,提高临床疗效及改善患者生活质量。60 patients with facial nerve injury caused by parotid operation were chosen to participate in the study on the treatment for the injury by Mongolian warm acupuncture in the Oral and Maxillofacial Surgery Department of the Affiliated Hospital of Inner Mongolia Medical University from January 2021 to June 2022.The patients were randomly divided into warm acupuncture treatment group,which accepted warm acupuncture treatment in Mongolian medicine combined with conventional treatment,and control observation group,which accepted conventional treatment only.The clinical efficacy,House⁃Brackmann facial nerve function grade,Compound Muscle Action Potential(CMAP),psychological status and quality of life(the MOS item short from health survey,SF⁃36 scale)were assayed to compare the effect of two kinds of treatments.The research results indicated that(1)the total effective rate in the warm acupuncture group was significantly higher than that in the control group(P<0.05);that(2)the level of House⁃Brackmann scale was significantly increased for two groups,however,most patients in the warm acupuncture treatment group were in gradeⅠand gradeⅡ,and there were significant statistical difference between the two groups(P<0.05);that(3)CMAP was higher and BR was shorter after treatment than that before treatment for two groups,which was more obvious in warm acupuncture treatment group(P<0.05);that(4)the psychological scores of the two groups increased after treatment for two groups,and the scores of warm acupuncture treatment group was significantly higher than that of the control observation group(P<0.05);that(5)the score of SF⁃36 scale in two groups increased after treatment for two groups,and the scores of warm acupuncture treatment group was significantly higher than that of the control observation group(P<0.05).Conclusively,the warm acupuncture treatment in Mongolian medicine was able significantly to improve the House⁃Brackmann facial nerve function grade and neuroemg index of patients with facial nerve injury cause
