黄连油纱条换药治疗热毒炽盛型肛痈术后创面愈合的临床效果

摘　　要：目的:分析黄连油纱条换药治疗热毒炽盛型肛痈术后创面愈合的临床疗效。方法:选取50例热毒炽盛型肛痈患者,随机数字表法分为试验组(25例)和对照组(25例),两组均采用椎管内麻醉下低位肛周脓肿切开引流的术式,手术当日至术后4日每天静脉滴注同种抗生素以抗感染,并使用止血药静脉滴注以预防创面出血,术后第2日起每日排出粪便后及换药前使用专科熏洗2号方进行肛门局部熏洗。此外,对照组25例使用凡士林纱条填塞创面换药,试验组25例采用黄连油纱条填塞创面换药。记录两组患者第7、14、21日的创面面积和创面愈合率,记录两组患者第3、7、10日的创面分泌物、疼痛情况,记录创面愈合时间和最终治疗效果。结果:两组总有效率均为100%(25/25),提示两组患者均得到治愈,术后第3、7、10日比较,两组患者术后创面分泌物、疼痛情况差异存在统计学意义(P <0.05)。两组患者术后第7、14、21日的创面面积和创面愈合率比较,差异存在统计学意义(P <0.05)。两组的创面最终愈合时间比较,差异存在统计学意义(P <0.05)。结论 :黄连油纱条换药治疗热毒炽盛型肛痈术后创面的疗效更好,对于缩小术后创面面积,减少术后创面分泌物,缩短创面愈合时间,提高术后创面愈合率,减轻术后创面疼痛,以及促进术后创面愈合等具有积极作用,且安全可靠,宜推广运用。Objective:To analyze the clinical effect of dressing change of Huang-Lian(黄连)oil gauze on wound healing of heat-toxic incandescent anal carbuncle after operation.Methods:A total of 50 patients with heat-toxic incandescent anal carbuncle were randomly divided into the experimental group(25 cases)and the control group(25 cases).The two groups were treated by incision and drainage of low perianal abscess under intraspinal anesthesia.The same antibiotics were injected intravenously every day from the day of surgery to 4 days after surgery to prevent infection,and hemostatic drugs were used to prevent wound bleeding.After excretion of feces from the second day after surgery and before dressing change,local anal fumigation was performed with specialist fumigation recipe No.2.In addition,25 patients in the control group were treated with vaseline gauze and 25 patients in the experimental group were treated with Huang-Lian oil gauze.The wound area and healing rate in the two groups on the 7th,14th and 21st day were recorded;the wound secretions and pain in the two groups on the 3rd,7th and 10th day were recorded;the wound healing time and the final therapeutic effect were recorded.Results:Both groups were cured and the total effective rate was 100%(25/25).There were significant differences in wound secretion and pain between the two groups on the 3rd,7th and 10th day after operation(P<0.05).There were significant differences in wound area and wound healing rate between the two groups on the 7th,14th and 21st day after operation(P<0.05),and there was a significant difference in the final wound healing time between the two groups(P<0.05).Conclusion:The application of Huang-Lian oil gauze in dressing change after anus carbuncle operation is bettter,it can reduce the area of wound,reduce the secretion of wound after operation,shorten the time of wound healing,increase the rate of wound healing,reduce the pain of wound,and promote wound healing.It is safe and reliable,and is worth popularizing.
