子宫前壁部分切除整形修复术治疗凶险性前置胎盘合并胎盘植入的疗效分析

摘　　要：目的探讨子宫前壁部分切除整形修复术治疗凶险性前置胎盘合并胎盘植入的临床疗效。方法选取43例凶险性前置胎盘合并胎盘植入患者,根据随机数字表法将患者分为研究组(23例)与对照组(20例)。对照组患者予以常规手术治疗,研究组患者予以子宫前壁部分切除整形修复术治疗。比较两组患者临床相关指标、子宫切除率及子宫切口愈合情况。结果研究组患者的手术时间、住院时间均短于对照组,术中及术后24h总失血量、输血量均少于对照组,差异有统计学意义(P<0.05)。研究组及对照组患者均无子宫切除,子宫切口愈合情况均为100%,差异无统计学意义(P>0.05)。结论在凶险性前置胎盘合并胎盘植入患者中采取子宫前壁部分切除整形修复术治疗,可有效改善临床指标,促进子宫切口愈合,疗效显著,值得临床推广与应用。Objective To investigate the clinical effect of partial resection of the anterior wall of uterus for the treatment of dangerous placenta previa complicated with placenta implantation.Methods 43 patients with dangerous placenta previa combined with placenta implantation were selected and divided into study group(23 cases)and control group(20 cases)according to random number table method.The control group was treated with conventional surgery,and the study group was treated with partial resection of the anterior uterine wall.The clinical indicators,the incidence of hysterectomy rate and the healing of uterine incision were compared between the two groups.Results The operation time and hospital stay of the study group were shorter than those of the control group,and the total blood loss and transfusion volume at 24 h after operation were lower than those of the control group,the difference was statistically significant(P<0.05).No hysterectomy was performed in both study group and control group,and the healing of uterine incision was 100%,with no statistical significance(P>0.05).Conclusion In the patients with dangerous placenta previa complicated with placenta implantation,partial resection of the anterior uterine wall can effectively improve the recovery of clinical indicators,promote the healing of uterine incision,and is worthy of clinical promotion and application.
