摘　　要：目的探讨卡前列素氨丁三醇联合宫颈提拉式缝合术在前置胎盘剖宫产难治性产后出血中的应用效果。方法选取40例前置胎盘剖宫产难治性产后出血患者为研究对象,随机分为两组,各20例。参照组实施缩宫素联合双侧子宫动脉下行支结扎术,试验组在参照组的基础上,实施卡前列素氨丁三醇联合宫颈提拉式缝合术,比较两组的止血效果、出血量及并发症发生情况。结果试验组止血总有效率为95.00%,高于参照组的70.00%,差异有统计学意义(P<0.05);试验组术中及术后2h、12h、24h出血量少于参照组,差异有统计学意义(P<0.05);试验组并发症发生率为10.00%,低于参照组的40.00%,差异有统计学意义(P<0.05)。结论卡前列素氨丁三醇联合宫颈提拉式缝合术在前置胎盘剖宫产难治性产后出血中的应用效果显著,能有效降低术中及术后出血量,减少并发症发生风险。Objective To exploring the application effect of combination of carboprost,tromethamine and cervical pullup suture in the treatment of refractory postpartum hemorrhage after cesarean section with placenta previa.Methods 40 patients with intractable postpartum hemorrhage after cesarean section with placenta previa were selected as the study subjects and randomly divided into two groups,with 20 cases in each group.The reference group was treated with oxytocin combined with bilateral uterine artery descending branch ligation,while the experimental group was treated with carboprost tromethamine combined with cervical pull-up suture on the basis of the reference group.Results The hemostatic effect,bleeding volume,and incidence of complications were compared between the two groups.The total effective rate of hemostasis in the experimental group was 95.00%,higher than 70.00%in the reference group,with a statistically significant difference(P<0.05);The intraoperative and postoperative bleeding volume of the experimental group at 2,12,and 24 hours was lower than that of the control group,with a statistically significant difference(P<0.05);The incidence of complications in the experimental group was 10.00%,lower than the reference group's 40.00%,with a statistically significant difference(P<0.05).Conclusion The combination of carboprost,aminobutanediol,and cervical pull-up suture has a significant effect on the treatment of refractory postpartum hemorrhage in cesarean section with placenta previa,which can effectively reduce intraoperative and postoperative bleeding,and reduce the risk of complications.
