摘　　要：目的:分析检验过程不合格标本出现的问题环节,并针对性地提出改进措施。方法:对南京市妇幼保健院检验科2021年1—10月接收到的标本不合格情况进行回顾性分析,按原因进行分类统计,并提出改进措施。结果:检验科接收到的标本共计2791135份,不合格标本1681份,样本不合格率为0.06%。其中就诊信息有误0.36%,开立医嘱不当16.78%,采集标本不当33.14%,运输标本不当0.36%。结论:培养护士采集标本责任心,严格执行三查七对制度;培训临床医生检验医嘱名称及对应检验项目的临床意义;针对收费窗口,加强管理,确保患者信息核对清楚;针对样本运送人员,定期培训考核,确保掌握各检验标本的运输保存方法及检验地点,确保标本及时合格地送到检验者手中。只有加强管理,强化培训,确保检验流程的每一个环节都高效准确地完成,才能保证检测结果的准确及时有效。Objective To find out the problem of unqualified specimens in the process of inspection,and put forward the improvement measures.Methods A retrospective analysis was conducted on the unqualified specimens received from the Clinical Laboratory of Nanjing Women and Children's Healthcare Hospital from January 2021 to October 2021,and classified statistics were made according to the causes,and improvement measures were put forward.Results A total of 2791135 specimens were received in the laboratory,including 1681 unqualified specimens,the unqualified rate of samples was 0.06%.Among the unqualified specimens,0.36%had incorrect medical information,16.78%had improper medical orders,33.14%had improper specimen collection,and 0.36%had improper specimen transportation.Conclusions To cultivate nurses'sense of responsibility for collecting specimens,and strictly implement the system of three inspections and seven verifications;to train clinicians on the names of each test order and the clinical significance of the corresponding test items;strengthen management of the charging window to ensure that patient information is checked clearly;For the sample transport personnel,regular training and assessment to ensure that the transportation and preservation methods and test locations of each test specimen are mastered,and to ensure that the specimens are timely and qualified to the hands of the inspectors.Only by strengthening management and training to ensure that every link of the inspection process is efficiently and accurately completed,can the accurate and effective test results be guaranteed.
