T_(2)-FLAIR增强联合T_(1)WI-3D-TFE增强序列诊断癌性脑膜炎的临床分析

摘　　要：目的:研究癌性脑膜炎应用T_(2)-FLAIR增强以及T_(1) WI-3D-TFE增强序列联合诊断的有效性。方法:将2019年6月~2023年6月在本院接受临床诊断的59例疑似癌性脑膜炎患者列入医学观察范畴,对全部患者进行T_(1) WI-3DTFE增强序列检查、T_(2)-FLAIR增强检查、T_(2)-FLAIR增强联合T_(1) WI-3D-TFE增强序列检查,分别为T_(1) WI-3DTFE增强序列组,T_(2)-FLAIR增强组,联合检查组,把最终结果视作金标准,统计癌性脑膜炎基本特征,评估诊断疾病的最佳方式。结果:临床诊断59例疑似患者,30例患者为癌性脑膜炎,占比50.85%,29例患者为非癌性脑膜炎、占比49.15%。患者病程4~7周,平均(5.02±0.09)周;T_(1) WI-3D-TFE增强序列组检出21例、占比70.00%,T_(2)-FLAIR增强组检出22例、占比73.33%,联合检查组检出28例、占比93.33%,比较之下联合检查组检出率更高,P<0.05;医师对联合检查组的检验认可度分数均高于T_(1) WI-3D-TFE增强序列组、T_(2)-FLAIR增强组,P<0.05。结论:诊断癌性脑膜炎,应意识到T_(2)-FLAIR增强联合T_(1) WI-3D-TFE增强序列诊断的重要作用和价值,提高疾病检出准确率。Objective:To study the effectiveness of T_(2)-FLAIR augmentation and T_(1) WI-3D-TFE augmentation sequence combination diagnosis in cancerous meningitis.Methods:59 cases of suspected cancerous meningitis in our hospital from June 2019 to June 2023 were included in the category of medical observation.All patients underwent T_(1) WI-3D-TFE enhancement sequence examination,T_(2)-FLAIR enhancement examination,T_(2)-FLAIR enhancement and T_(1) WI-3D-TFE enhancement sequence examination,which were recorded as T_(1) WI-3D-TFE enhancement sequence examination group,T_(2)-FLAIR enhancement examination group and joint examination group,regarded the final result as gold standard,counted the basic characteristics of cancerous meningitis,and evaluated the best way to diagnose the disease.Results:Clinical diagnosis of 59 suspected patients,30 patients were cancerous meningitis,accounting for 50.85%,and 29 patients were non-cancerous meningitis,accounting for 49.15%.The duration of patients was between 4 and 7 weeks,with mean value(5.02±0.09)weeks;21 cases,70.00%,22 cases,73.33%,28 cases,93.33%,the combined examination group was higher,P<0.05;The approval score of the combined examination group was higher than T_(1) WI-3D-TFE enhancement sequence examination group and T_(2)-FLAIR enhancement examination group,P<0.05.Conclusion:In the diagnosis of cancerous meningitis,we should realize the important role and value of T_(2)-FLAIR enhancement combined with T_(1) WI-3D-TFE to enhance sequence diagnosis,and improve the accuracy of disease detection.
