摘 要:目的:分析全自动血细胞分析仪联合血涂片细胞形态学在血常规检验中的价值。方法:选取2021年5月~2023年5月于本院行血液检查的96例患者,采集其血液标本,均行全自动血细胞分析仪、血涂片细胞形态学检查,以复检结果为“金标准”,分析两种检测方法单独、联合在血常规检验中的效能。结果:以复检结果为“金标准”,96份血液标本中,阳性75份,阴性21份;全自动血细胞分析仪共检出66份阳性,血涂片细胞形态学共检出65份阳性,全自动血细胞分析仪联合血涂片细胞形态学共检出74份阳性;全自动血细胞分析仪联合血涂片细胞形态学检查的灵敏度为97.33%(73/75)、准确度为95.83%[(73+20)/96]、阴性预测值为90.91%(20/22),高于两者单独检查的85.33%(64/75)、86.46%[(64+19)/96]、63.33%(19/30)、84.00%(63/75)、85.42%[(63+19)/96]、61.29%(19/31),差异有统计学意义(P<0.05);三者特异度、阳性预测值相比,差异无统计学意义(P>0.05);Kappa检验显示:全自动血细胞分析仪联合血涂片细胞形态学检查结果与“金标准”检查结果的一致性极好(Kappa值=0.911,P=0.000)。结论:全自动血细胞分析仪联合血涂片细胞形态学在血常规检验中效果显著,可提升检验的准确度。Objective:To analyze the value of automatic blood cell analyzer combined with blood smear cell morphology in blood routine test.Methods:96 patients who underwent blood tests in our hospital from May 2021 to May 2023 were selected,and their blood samples were collected.All patients underwent automatic blood cell analyzer and blood smear cell morphology examination,and the retest results were used as the“gold standard”to analyze the efficacy of the two detection methods alone and in combination in blood routine testing.Results:Among 96 blood samples,75 were positive and 21 were negative;A total of 66 positive samples were detected by automatic blood cell analyzer,65 positive samples were detected by blood smear cell morphology,and 74 positive samples were detected by automatic blood cell analyzer combined with blood smear cell morphology;The sensitivity,accuracy and negative predictive value of automatic blood cell analyzer combined with blood smear cell morphology examination were 97.33%(73/75),95.83%[(73+20)/96]and 90.91%(20/22),respectively,which were higher than 85.33%(64/75),86.46%[(64+19)/96],63.33%(19/30),84.00%(63/75),85.42%[(63+19)/96]and 61.29%(19/31)respectively(P<0.05);There was no significant difference in specificity and positive predictive value among the three groups(P>0.05);Kappa test showed that the results of automatic blood cell analyzer combined with blood smear cell morphology were in good agreement with the results of“gold standard”(Kappa=0.911,P=0.000).Conclusion:The effect of automatic blood cell analyzer combined with blood smear cell morphology in blood routine test is significant,which can improve the accuracy of the test.
