内镜黏膜下剥离术治疗贲门癌早期及癌前病变的疗效及安全性分析

摘　　要：目的:分析贲门癌早期与癌前病变治疗中用内镜黏膜下剥离术治疗的临床疗效及安全性。方法:将2018年1月~2022年12月莆田学院附属医院消化内科收治的贲门癌早期及癌前病变患者共112例作为本次观察对象,所有患者均经临床检查确诊后采取内镜黏膜下剥离术治疗;统计分析患者治疗后临床疗效、病灶完整切除率、复发率及并发症发生率(出血、穿孔、贲门狭窄、发热)。结果:112例患者经内镜黏膜下剥离术治疗后可见,治疗总有效人数为108例,治疗总有效率为96.43%,另有4例患者治疗效果并不明显,给予对症治疗;本次病灶完整切除人数为108例,病灶完整切除率为96.43%;治疗后随访6月可见并无患者出现疾病复发或转移;治疗后统计并发症可见,术后出血3例,术中穿孔1例,术后贲门狭窄5例,术后发热11例,总发生率为17.85%。结论:对贲门癌早期及癌前病变患者采取内镜黏膜下剥离术治疗可显著提高疾病治疗效果与病灶完整切除率,有助于降低术后并发症发生率与复发率,保证手术治疗安全性。Objective:To analyze the clinical efficacy and safety of endoscopic submucosal dissection in the treatment of early gastric cancer and precancerous lesions.Methods:A total of 112 patients with early and precancerous lesions of cardia cancer admitted to the Department of Gastroenterology at Putian University Affiliated Hospital from January 2018 to December 2022 were selected as the observation subjects.All patients were diagnosed through clinical examination and treated with endoscopic submucosal dissection;Statistical analysis of clinical efficacy,complete lesion resection rate,recurrence rate,and incidence of complications(bleeding,perforation,cardiac stenosis,fever)after treatment in patients.Results:112 patients were treated with endoscopic submucosal dissection,with a total effective rate of 96.43%and a total effective number of 108 cases.In addition,4 patients did not have significant treatment effects and were given targeted treatment;The number of patients with complete lesion resection this time was 108,with a complete lesion resection rate of 96.43%;After 6 months of follow-up after treatment,it can be seen that no patients have experienced disease recurrence or metastasis;After treatment,complications were observed in statistics,including 3 cases of postoperative bleeding,1 case of intraoperative perforation,5 cases of postoperative cardiac stenosis,and 11 cases of postoperative fever.The total incidence rate was 17.85%.Conclusion:Endoscopic submucosal dissection can significantly improve the treatment effect and complete lesion resection rate for patients with early gastric cancer and precancerous lesions,help reduce the incidence of postoperative complications and recurrence,and ensure the safety of surgical treatment.
