凉血散瘀汤结合常规西药治疗IgA肾病疗效及对患者血液流变学、炎性细胞因子水平的影响

摘　　要：目的:分析凉血散瘀汤结合常规西药治疗IgA肾病对患者血液流变学、炎性细胞因子水平的影响。方法:选入96例IgA肾病患者,随机分为治疗组(48例,常规西药+凉血散瘀汤治疗)、对照组(48例,常规西药治疗),比较总有效率,治疗前后血液流变学指标(全血高切黏度、全血低切黏度)、血清炎性细胞因子[单核细胞趋化因子(MCP-1)、白介素-6(IL-6)]、肾功能指标[血尿酸(BUA)、血肌酐(Scr)]组间比较,统计给药期间患者消化道症状、神经系统症状、四肢及躯干症状等不良反应。结果:治疗总有效率比较,治疗组高于对照组(P<0.05);治疗后全血高切、低切黏度比较,治疗组低于对照组;治疗后血清MCP-1、IL-6水平比较,治疗组低于对照组(P<0.05);治疗后治疗组BUA、Scr水平显著低于对照组(P<0.05);治疗组四肢及躯干症状、神经系统症状等不良反应总发生率与对照组比较无统计学差异(P>0.05)。结论:给予IgA肾病患者凉血散瘀汤结合常规西药治疗可显著改善其血液流变学,下调炎性细胞因子水平,发挥肾功能保护作用,整体疗效显著且安全性高。Objective:Analyze the effect of Liangxue Sanyu decoction combined with conventional western medicine in the treatment of IgA nephropathy on the blood rheology and inflammatory cytokine levels of patients.Methods:96 cases of IgA nephropathy were selected and all were treated with conventional western medicine.The treatment group was given Liangxue Sanyu decoction.The total effective rate was compared and the hemorrheological indicators were completed(whole blood high shear viscosity,whole blood low shear viscosity),serum inflammatory cytokines(monocyte chemotactic factor[MCP-1],interleukin-6[IL-6)]),renal function indicators(blood uric acid[BUA],serum creatinine[Scr])were compared between groups,and adverse reactions such as gastrointestinal symptoms,neurological symptoms,limbs and trunk symptoms of patients during the administration period were counted.Results:The therapeutic effect of the treatment group was better than that of the control group(P<0.05).Comparing the high-shear and low-shear viscosity of whole blood after treatment,the treatment group was lower(P<0.05).Comparison of IL-6,MCP-1,Scr,and BUN levels between groups,the treatment group was lower(P<0.05).Comparing the total incidence of adverse reactions such as trunk symptoms and neurological symptoms,there was no significant difference between groups(P>0.05).Conclusion:Liangxue Sanyu decoction combined with conventional western medicine treatment to patients with IgA nephropathy can significantly improve their hemorrheology,reduce the levels of inflammatory cytokines,and exert a protective effect on renal function.The overall effect is significant and safe.
