膳食调整、规律运动训练结合呼吸训练在糖尿病合并COPD患者中的应用效果

摘  要:目的探讨膳食调整、规律运动训练结合呼吸训练对糖尿病合并慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者的血糖水平影响。方法选取2018年1月—2021年1月在武夷山市立医院呼吸内科住院治疗的93例2型糖尿病(diabetes mellitus type 2,T2DM)合并COPD患者为研究对象,基于随机数表法分成对照组(47例)和研究组(46例)。两组均行对症治疗,对照组遵医用降糖药及常规干预,研究组用降糖药同时配合膳食调整、规律运动训练结合呼吸训练,3个月后测定空腹血糖(fasting blood glucose,FPG)、餐后2 h血糖(2-hour postprandial plasma glucose,2 hPG)及糖化血红蛋白(glycosylated hemoglobin antibody,HbA1c)水平,掌握HbA1c达标和复合终点达标情况。结果干预后,研究组FPG、2 hPG及HbA1c显著低于对照组,差异有统计学意义(P<0.05)。研究组的HbA1c达标率、复合终点达标率均显著高于对照组,差异有统计学意义(P<0.05)。结论在T2DM合并COPD患者治疗中配合膳食调整、规律运动训练结合呼吸训练等干预,可有效改善和控制血糖水平,有着重要临床意义。Objective To explore the effects of dietary adjustment,regular exercise training combined with respiratory training on blood glucose level in patients with diabetes mellitus complicated with chronic obstructive pulmonary dis-ease(COPD).Methods 93 patients with type 2 diabetes mellitus(T2DM)combined with COPD hospitalized in the De-partment of Respiratory Medicine of Wuyishan Municipal Hospital from January 2018 to January 2021 were selected as the study subjects,and were divided into control group(47 cases)and study group(46 cases)based on random num-ber table method.The two groups were treated with symptomatic treatment,the control group was treated with medical hypoglycemic drugs and routine intervention,and the study group was treated with hypoglycemic drugs combined with dietary adjustment,regular exercise training and breathing training.Fasting blood glucose(FPG),2-hour postprandial plasma glucose(2 hPG)and glycosylated hemoglobin antibody(HbA1c)levels were measured 3 months later,and HbA1c compliance and composite endpoint compliance were mastered.Results After intervention,FPG,2 hPG and HbA1c in the study group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The HbA1c compliance rate and the composite endpoint compliance rate of the study group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion In the treatment of T2DM patients with COPD,dietary adjustment,regular exercise training combined with breathing training and other interventions can effectively improve and control blood glucose levels,which has im-portant clinical significance.
