2型糖尿病尿液培养阳性患者的病原菌分布与耐药情况分析

摘  要:目的掌握2型糖尿病合并尿路感染阳性患者的病原菌分布情况,了解其对药物的耐药性情况。方法回顾性分析2020年11月—2022年12月福建省三明市沙县区总医院收治的90例2型糖尿病合并尿路感染患者的临床资料,并设置为观察组。另选择同期本院泌尿科以及肾内科收治的90例单纯尿路感染患者的临床资料为对照组。对两组患者均进行病原菌分布检测,同时对药物敏感性进行检测。结果90例2型糖尿病患者共培养尿液标本病原菌100株,对照组共培养尿液标本病原菌100株。观察组培养尿液标本阳性检出以革兰阴性菌为主,共培养70株,其中大肠埃希菌占比最高,其次为肺炎克雷伯菌。两组革兰阳性球菌、真菌以及支原体感染构成比对比,差异有统计学意义(P<0.05)。结论2型糖尿病患者应高度重视尿路感染的可能性,合并尿路感染患者近年来发病率较高,病原菌以大肠埃希菌为主,从耐药性角度上进行考虑应高度重视革兰阴性杆菌对碳青霉烯类药物的耐药情况,强化临床细菌培养以及药物敏感性实验,结合临床结果合理用药,确保临床效果。Objective To grasp the pathogenic bacteria distribution of positive patients with type 2 diabetes mellitus combined with urinary tract infection,and to understand their resistance to drugs.Methods The clinical data of 90 pa-tients with type 2 diabetes mellitus complicated with urinary tract infection admitted to Shaxian District General Hos-pital of Sanming City,Fujian Province from November 2020 to December 2022 were retrospectively analyzed and set as the observation group.In addition,the clinical data of 90 patients with simple urinary tract infection admitted to the urology department and nephrology department of our hospital during the same period were selected as the control group.The distribution of pathogenic bacteria and drug sensitivity were detected in both groups.Results 90 patients with type 2 diabetes mellitus were co-cultured with 100 strains of pathogenic bacteria in urine samples,and 100 strains of pathogenic bacteria in urine samples were co-cultured in the control group.The positive detection of urine samples in the observation group was mainly Gram-negative bacteria,and a total of 70 strains were cultured,of which Escherichia coli accounted for the highest proportion,followed by Klebsiella pneumoniae.Comparison of infection composition ratios between two groups of Gram positive cocci,fungi,and mycoplasma,and the difference was statisti-cally significant(P<0.05).Conclusion Type 2 diabetes mellitus patients should pay great attention to the possibility of urinary tract infection,combined with the high incidence of urinary tract infection patients in recent years,the patho-genic bacteria with Escherichia coli as the main,from the perspective of drug resistance to consider due to pay great attention to Gram-negative bacilli on the carbapenem drug resistance,strengthen the clinical bacterial culture and drug susceptibility experiments,combined with the clinical results of the rational use of medication,to ensure the clinical effect.
