摘　　要：目的研究疼痛管理模式下不同绷带包扎时间对全膝关节置换术后患者早期功能恢复的影响。方法选择2022年6月至2023年5月单侧全膝关节置换手术(TKA)患者80例,根据弹力绷带使用时间,24 h为对照组(40例),48 h为研究组(40例);观察两组围手术期术侧下肢的周径、膝关节功能、伤口并发症、相关不良反应,随访期间步速、步频、步幅。结果术后7 d、14 d大腿、膝关节、小腿周径均短于对照组;术后7 d、30 d膝关节活动度(ROM)屈、伸均大于对照组,膝关节功能评分(HSS)高于对照组;术后3个月两组均出现好转,研究组步速、步频、步幅均高于对照组(P<0.05)。两组伤口并发症以及相关不良反应之间无统计学差异(P>0.05)。结论全膝关节置换术后患者在疼痛管理模式下,使用48 d弹力绷带,能够减轻术肢肿胀,增加肢体的屈伸活动,有利于膝关节功能恢复。Objective To study the effect of different bandage wrapping time on early functional recovery of patients after total knee arthroplasty under pain management mode.Methods A total of 80 patients with unilateral total knee arthroplasty(TKA)were selected from June 2022 to May 2023,and according to the time of elastic bandage application,24 h was the control group(40 cases),and 48 h was the study group(40 cases);circumference of the lower limb on the perioperative side of the operated limb,knee joint function,wound complications,related adverse reactions,and pace,frequency and length of step during the follow-up period were observed in the two groups.Results The circumference of the thigh,knee and calf were shorter than that of the control group at 7 d and 14 d postoperatively;the knee mobility(ROM)flexion and extension were greater than that of the control group at 7 d and 30 d postoperatively,and the knee function scores(HSS)were higher than that of the control group;at 3 months postoperatively,both groups showed improvement,and the study group's stride speed,stride frequency,and stride amplitude were higher than that of the control group(P<0.05).There was no statistical difference between the wound complications and related adverse reactions in the two groups(P>0.05).Conclusions The use of 48 d elastic bandage in patients after total knee arthroplasty under pain management mode can reduce the swelling of the operated limb,increase theflexion and extension activities of the limb,and is favorable to the recovery of knee joint function.
