急诊CRAMS评分结合分级转运护理在多发性创伤患者中的应用

摘　　要：目的研究分析急诊循环、呼吸、腹部、运动、语言(CRAMS)评分结合分级转运护理在多发性创伤患者中的应用效果。方法选取2022年7月至2023年7月福建医科大学附属第一医院急诊接受治疗的多发性创伤患者100例作为研究对象,依据抛硬币法对患者进行分组,分为对照组与观察组,各50例。对照组实施常规急诊护理,观察组给予急诊CRAMS评分结合分级转运护理。对两组伤情评估正确率、急救成功率、急救时效、心理状态、不良事件发生率、护理满意度进行分析比较。结果相较于参照组,观察组伤情评估正确率、急救成功率更高(P<0.05)。与对照组比较,观察组就诊等待时间、急救检查时间、急诊停留时间更短(P<0.05)。相较于护理前,两组护理后心理状态各指标评分明显降低(P<0.05);在护理后,观察组心理状态各指标评分明显低于对照组(P<0.05)。相比于对照组,观察组不良事件发生率更低,护理满意度更高(P<0.05)。结论急诊CRAMS评分结合分级转运护理有助于正确评估多发性创伤患者的伤情,可明显提高急救时效与急救成功率,改善心理状态,减少不良事件的发生,加快患者病情康复。Objective To study and analyze the application effect of emergency circulatory,respiratory,abdominal,motor,and language(CRAMS)scoring combined with graded transport nursing in patients with multiple trauma.Methods 100 patients with multiple trauma who received emergency treatment in the First Affiliated Hospital of Fujian Medical University were selected as the study subjects,with a time range of July 2022 to July 2023.The patients were divided into control group and observation group,50 cases in each group,according to the coin toss method.The control group received routine emergency care,while the observation group received emergency CRAMS scores combined with graded transfer care.The accuracy of injury assessment,success rate offirst aid,effectiveness offirst aid,psychological status,incidence of adverse events,and nursing satisfaction were statistically compared between the two groups.Results Compared with the reference group,the observation group had a higher accuracy in injury assessment and success rate in emergency treatment(P<0.05).Compared with the reference group,the observation group had shorter waiting time for medical treatment,emergency examination time,and emergency stay time(P<0.05).Compared to before nursing,the scores of various indicators of psychological state in the two groups decreased significantly after nursing(P<0.05);After nursing,the scores of various indicators of psychological state in the observation group were significantly lower than those in the control group(P<0.05).Compared with the reference group,the observation group had a lower incidence of adverse events and higher nursing satisfaction(P<0.05).Conclusions The combination of emergency CRAMS score and graded transportation nursing can help to correctly evaluate the injury situation of patients with multiple trauma,significantly improve the effectiveness and success rate of emergency treatment,improve psychological status,reduce the occurrence of adverse events,and accelerate patient recovery.
