摘  要:目的:观察腕掌指关节可动性计量矫形器在手腕部骨折术后功能障碍患者康复训练中的应用效果。方法:选取2020年6月至2021年12月该院收治的93例手腕部骨折患者进行前瞻性研究,按照随机数字表法将其分成观察组(n=47)和对照组(n=46)。观察组采用腕掌指关节可动性计量矫形器进行康复训练,对照组采用常规动态腕指矫形器进行康复训练,比较两组训练前、训练1个月后腕关节功能(Gartland-Werley腕关节功能评分表)评分、手腕肌力分级、腕部正中神经损害恢复程度(Chen W-S腕部正中神经损害)评分和生命质量[生活质量综合评定问卷(GQOLI-74)]评分。结果:训练后,观察组Gartland-Werley腕关节功能评分低于对照组,差异有统计学意义(P<0.05);观察组手腕肌力分级优于对照组,差异有统计学意义(P<0.05);观察组疼痛程度Chen W-S腕部正中神经损害评分低于对照组,两点辨别觉、大鱼际肌力、生活自理能力等Chen W-S腕部正中神经损害评分均高于对照组,差异有统计学意义(P<0.05);观察组心理功能、社会功能、躯体功能和物质生活等GQOLI-74评分均高于对照组,差异有统计学意义(P<0.05)。结论:腕掌指关节可动性计量矫形器应用于手腕部骨折术后功能障碍患者康复训练中可提高手腕肌力和生命质量评分,改善腕部正中神经损害恢复程度评分,降低腕关节功能评分,效果优于常规动态腕指矫形器康复训练。Objective:To observe application effects of wrist-metacarpophalangeal joint mobility measurement orthosis in rehabilitation training of patients with dysfunction after wrist fracture surgery.Methods:A prospective study was conducted on 93 patients with hand and wrist fractures admitted to the hospital from June 2020 to December 2021.They were divided into observation group(n=47)and control group(n=46)according to the random number table method.The observation group was treated with wrist-metacarpophalangeal joint mobility measurement orthosis rehabilitation training,while the control group was treated with conventional dynamic wrist orthosis rehabilitation training.The wrist function(Gartland-Werley wrist function score table)score,the wrist muscle strength grade,the wrist median nerve damage recovery degree(Chen W-S wrist median nerve damage)score,and the quality of life[general quality of life inventory(GQOLI-74)]score were compared between the two groups before and 1 month after the training.Results:After the training,the Gartland-Werley wrist function score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The wrist muscle strength grade of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The pain degree of Chen W-S wrist median nerve damage score of the observation group was lower than that of the control group,the Chen W-S wrist median nerve damage scores such as two-point discrimination,thenar muscle strength,and self-care ability were higher than those of the control group,and the differences were statistically significant(P<0.05).Further,the GQOLI-74 scores of psychological function,social function,physical function and material life in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The application of wrist-metacarpophalangeal joint mobility measurement orthosis in the reha
