芪参益气滴丸联合阿替洛尔与胺碘酮治疗冠心病合并心律失常患者的效果

摘　　要：目的:探讨芪参益气滴丸联合阿替洛尔与胺碘酮治疗冠心病合并心律失常患者的效果。方法:选取2020年1月至2023年2月该院收治的152例冠心病合并心律失常患者进行前瞻性研究,按照随机数字表法分为观察组和对照组各76例。对照组采用阿替洛尔联合胺碘酮治疗,观察组在对照组基础上联合芪参益气滴丸治疗。治疗12周后,比较两组临床疗效,治疗前后心律失常指标[24 h动态心电图室性期前收缩次数、短暂阵发性室性心动过速(短阵室速)次数、QTc间期]水平、血清炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞分化抗原40配体(CD40L)]水平、血管内皮功能指标[内皮素-1(ET-1)、一氧化氮(NO)、血管性血友病因子(vWF)]水平,以及不良反应发生率。结果:观察组治疗总有效率为94.74%(72/76),高于对照组的80.26%(61/76),差异有统计学意义(P<0.05);治疗12周后,观察组室性期前收缩次数、短阵室速次数均少于对照组,QTc间期短于对照组,血清IL-6、TNF-α、CD40L、vWF、ET-1水平均低于对照组,NO水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:芪参益气滴丸联合阿替洛尔与胺碘酮治疗冠心病合并心律失常患者效果确切,能改善24 h动态心电图指标,减轻血管内皮功能损伤,降低炎性因子水平,效果优于单用阿替洛尔与胺碘酮治疗。Objective:To investigate effects of Qishen Yiqi dropping pills combined with Atenolol and Amiodarone on patients with coronary heart disease complicated with arrhythmia.Methods:A prospective study was conducted on 152 patients with coronary heart disease and arrhythmia admitted to this hospital from January 2020 to February 2023.They were divided into observation group and control group according to the random number table method,76 cases in each group.The control group was treated with Atenolol combined with Amiodarone,while the observation group was treated with Qishen Yiqi dropping pills on the basis of those of the control group.After 12 weeks of treatment,the clinical efficacy,the levels of arrhythmia indexes[24 h dynamic electrocardiogram premature ventricular contraction number,short paroxysmal ventricular tachycardia number(short ventricular tachycardia),QTc interval]levels,the serum inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),leukocyte differentiation antigen 40 ligand(CD40 L)]levels,the vascular endothelial function indexes[endothelin-1(ET-1),nitric oxide(NO),von Willebrand factor(vWF)]levels,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 94.74%(72/76),which was higher than 80.26%(61/76)of the control group,and the difference was statistically significant(P<0.05).After 12 weeks of treatment,the premature ventricular contraction number and the short ventricular tachycardia number in the observation group were less than those in the control group,and the QTc interval was shorter than that in the control group;the levels of serum IL-6,TNF-α,CD40L,vWF and ET-1 were lower than those in the control group;the NO level was higher than that of the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Qishen Yiqi dropping pills combined with Atenolol a
