摘　　要：保障疾病冲击下的家庭福利水平是突破纾困扶弱的局限和完善收入分配格局的关键。本文使用CHARLS数据,通过事件研究法研究住院冲击对于经济与健康等家庭福利的影响。结果显示,住院冲击在短期内显著增加自付医疗支出,自付医疗支出的增长导致家庭支出压力加重。住院冲击降低患者本人和配偶的就业概率和收入水平,导致家庭总消费水平持续下降,而其下降主要集中在食品消费。住院冲击显著降低了患者自评健康水平,显著提高本人和配偶抑郁得分,持续降低了患者的生活满意度,并且对配偶的生活满意度有长期的负面影响。私人转移支付在缓解住院健康冲击方面发挥主要作用,而公共转移支付的作用目前仍十分有限。To guarantee the household welfare under the impact of disease is the key to break through the limitation of aid and help the weak and improve the pattern of income distribution.Using CHARLS(2011—2018),the paper uses the event study method to analyses the impact of hospitalization shock on household welfare,including income,labor,consumption health behavior,health and psychological status of patients and their spouse.The results show that the hospitalization shock significantly increases the out-of-pocket medical expenditure in the short term,and the increase of out-of-pocket medical expenditure leads to household expenditure pressure.The shock of hospitalization reduces the employment probability and income of patients and their spouses,resulting in a decrease in total household consumption in both the short and long term.The impact of hospitalization significantly decreased self-rated health,significantly increased the depression scores of self and spouse,and continuously reduced both the life satisfaction,while it causes negative impact on the life satisfaction of spouse in long term.Private transfer plays an important role in alleviating the health shock of hospitalization,but public transfer payment is still very limited.
