血清MMP-2、TNF-α及IL-17在脊柱结核患者预后中的价值研究

摘　　要：目的评价血清基质金属蛋白酶2(matrix metalloproteinase-2,MMP-2)、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、白细胞介素-17(interleukin-17,IL-17)在脊柱结核患者预后中的价值。方法选取2021年1月—2023年6月广州市胸科医院收治的60例脊柱结核患者为观察组,根据预后水平将其分为预后良好组(n=30)、预后不良组(n=30),并于同期选取30例健康体检者为对照组。对比分析观察组与对照组间、预后良好组与预后不良组间的血清MMP-2、TNF-α、IL-17水平差异。结果观察组患者的血清MMP-2(31.86±3.57)pg/mL、TNF-α(19.78±6.12)pg/mL、IL-17(40.18±6.40)pg/mL,高于对照组,差异有统计学意义(t=7.319、10.814、4.334,P<0.05)。预后不良血清组MMP-2、TNF-α、IL-17水平均高于预后良好组,差异有统计学意义(P<0.05)。结论脊柱结核患者的血清MMP-2、TNF-α、IL-17水平相较于健康人群会有明显的升高,且指标参数的上升情况与患者预后水平息息相关。Objective To evaluate the value of serum matrix metalloproteinase-2(MMP-2),tumor necrosis factor-α(TNF-α),and interleukin-17(IL-17)in the prognosis of patients with spinal tuberculosis.Methods Sixty patients with spinal tuberculosis admitted to Guangzhou Chest Hospital from January 2021 to June 2023 were selected as the observation group,which were categorized into good prognosis group(n=30)and bad prognosis group(n=30)according to the prognostic level,and 30 healthy medical checkups were selected in the same period as the control group.The differences in serum MMP-2,TNF-α,and IL-17 levels between the observation group and the control group,the good prognosis group and the bad prognosis group were comparatively analyzed.Results The serum MMP-2(31.86±3.57)pg/mL,TNF-α(19.78±6.12)pg/mL and IL-17(40.18±6.40)pg/mL in the observation group were higher than those in the control group,and the differences were statistically significant(t=7.319,10.814,4.334,P<0.05).The levels of MMP-2,TNF-αand IL-17 in bad prognosis group were higher than those in good prognosis group,and the difference was statistically significant(P<0.05).Conclusion Serum MMP-2,TNF-α,and IL-17 levels in spinal tuberculosis patients are significantly higher compared with those in the healthy population,and the rise in the index parameters is closely related to the prognosis level of patients.
