复方利多卡因乳膏局部涂抹对无痛静脉穿刺患儿疼痛和穿刺成功率的影响

摘　　要：目的探究复方利多卡因乳膏局部涂抹对无痛静脉穿刺患儿疼痛和穿刺成功率的影响。方法将2022年8月—2023年3月于清远市妇幼保健院儿童呼吸科接受静脉输液的400例患儿作为研究对象。将穿刺前接受常规消毒的患儿纳入常规消毒组(197例),将穿刺前接受复方利多卡因乳膏局部涂抹的患儿纳入利多卡因组(203例)。比较两组穿刺疼痛程度、穿刺成功率、躯体应激程度及穿刺不良事件发生情况。结果利卡多因组患儿面部表情评定法评分(2.45±0.57)分低于常规消毒组的(5.51±1.12)分,差异有统计学意义(t=34.585,P<0.05)。利卡多因组患儿穿刺成功率高于常规消毒组,差异有统计学意义(P<0.05)。利多卡因组患儿穿刺时心率、收缩压、舒张压水平均低于常规消毒组,差异有统计学意义(P<0.05)。利卡多因组患儿穿刺不良事件发生率低于常规消毒组,差异有统计学意义(P<0.05)。结论局部涂抹复方利卡多因乳膏能够缓解患儿静脉穿刺时的疼痛感,提高穿刺成功率,降低躯体应激水平和穿刺不良事件发生率。Objective To investigate the effect of topical application of compound lidocaine cream on pain and puncture success rate of children with painless venipuncture.Methods 400 children who received intravenous infusion in the Department of Pediatric Respiratory Medicine of Qingyuan Maternal and Child Healthcare Hospital from August 2022 to March 2023 were used as study subjects.Children who received routine disinfection before puncture were included in the routine disinfection group(197 cases),and children who received topical application of compound lidocaine cream before puncture were included in the lidocaine group(203 cases).The degree of puncture pain,the success rate of puncture,the degree of physical stress and the occurrence of puncture adverse events were compared between the two groups.Results The facial expression evaluation score of the Ricadoine group[(2.45±0.57)points]was lower than that of the conventional disinfection group[(5.51±1.12)points],and the difference was statistically significant(t=34.585,P<0.05).The success rate of puncture in Ricadoine group was higher than that in conventional disinfection group,and the difference was statistically significant(P<0.05).The heart rate,systolic blood pressure and diastolic blood pressure in lidocaine group were lower than those in conventional disinfection group,and the differences were statistically significant(P<0.05).The incidence of adverse events in the Ricadoine group was lower than that in the conventional disinfection group,and the difference was statistically significant(P<0.05).Conclusion Topical application of compound lidocaine cream can alleviate the pain during venipuncture,improve the success rate of venipuncture,and reduce the level of somatic stress and the incidence of puncture adverse events.
