摘 要:目的研究乙型肝炎患者在不同乙肝五项模式下血清乙肝病毒脱氧核糖核酸(hepatitis B virus deoxyri‐bonucleic acid,HBV-DNA)检测结果与其临床意义。方法选取2021年2月—2023年1月在聊城市传染病医院收治的100例乙型肝炎患者为研究对象。患者均进行血清标志物检测和HBV-DNA的检测,依据血清标志物检测结果将其分为5组,A组31例(“大三阳”)、B组44例(“小三阳”)、C组13例(抗-HBs、抗-HBe、抗-HBc阳性)、D组10例(HBsAg、抗-HBc阳性)、E组2例(抗-HBs阳性)。并比较不同血清标志物检测结果下患者的HBV-DNA含量,分析HBeAg表达患者和未表达患者之间HBV-DNA含量的不同。结果5组患者HBV-DNA含量比较,差异有统计学意义(P<0.001)。HBeAg表达患者的HBV-DNA含量为(750.20±80.29)拷贝/mL,高于HBeAg未表达患者的(452.72±75.19)拷贝/mL,差异有统计学意义(t=17.917,P<0.001)。经过Pearson相关性分析结果显示,HBV-DNA浓度与HBeAg检出率存在正相关关系(r=0.132,P<0.05)。结论不同乙肝五项模式及HBeAg阳性患者和阴性患者的HBV-DNA含量不同,临床中应该积极检测乙肝五项和HBV-DNA,有助于辨别乙型肝炎的分型,并为临床治疗提供可靠依据。Objective To study the results of serum hepatitis B virus deoxyribonucleic acid(HBV-DNA)detection and its clinical significance in patients with hepatitis B under different hepatitis B five patterns.Methods 100 pa‐tients of hepatitis B patients admitted to Liaocheng Infectious Disease Hospital from February 2021 to January 2023 were selected as the observation subjects.The patients were tested for serum markers and HBV-DNA.They were di‐vided into 5 groups according to the results of serum marker tests,31 cases in group A("major triple positive"),44 cases in group B("minor triple positive"),13 cases in group C(anti-HBs,anti-HBe,anti-HBc positive),and 10 cases in group D(HBsAg,anti-HBc positive),2 cases in group E(anti-HBs positive).The HBV-DNA content of patients with different serum marker test results were compared,and the difference in HBV-DNA content between HBeAgexpressing and non-expressing patients was analyzed.Results The HBV-DNA levels of patients in the 5 groups were compared,and the differences were statistically significant(P<0.001).The HBV-DNA content of HBeAg-expressing patients was(750.20±80.29)copies/mL,which was higher than(452.72±75.19)copies/mL of HBeAg non-expressing patients,and the difference was statistically significant(t=17.917,P<0.001).After Pearson correlation analysis,the re‐sults showed that there was a positive correlation between HBV-DNA concentration and HBeAg detection rate(r=0.132,P<0.05).Conclusion Different hepatitis B five patterns and HBV-DNA levels in HBeAg-positive and negative patients were different,and hepatitis B pentameter and HBV-DNA should be actively detected in the clinic,which can help to identify hepatitis B subtypes,and provide a reliable basis for clinical treatment.
