中药熏洗结合康复训练治疗脑卒中后偏瘫肢体痉挛的临床效果与分析

摘　　要：目的分析探讨中药熏洗结合康复训练治疗脑卒中后偏瘫肢体痉挛的临床效果。方法选取2021年7月—2023年7月南宁市第二人民医院中医科收治的脑卒中后偏瘫肢体痉挛患者80例为研究对象,采取病案号随机分组法分为观察组与对照组,每组40例。对照组给予中药熏洗,观察组在对照组的基础上添加康复训练。对比分析两组下肢血流速度、下肢周径、血液流变指标、深静脉血栓及下肢肿胀发生率、生活能力评分以及Barthel评分与Lovett分级情况。结果观察组下肢血流速度(35.86±4.61)cm/s,下肢周径(30.25±2.11)cm,优于对照组的(24.95±4.16)cm/s、(33.14±2.03)cm,差异有统计学意义(t=10.994、4.086,P<0.05)。观察组血液流变指标、深静脉血栓及下肢肿胀发生率、Barthel评分与Lovett分级均优于对照组,差异有统计学意义(P<0.05)。结论针对脑卒中后偏瘫肢体痉挛患者应用中药熏洗与康复训练能改善血液流变学,提升血流速度,有效预防术后形成深静脉血栓,切实改善生活能力,整体安全性较高。Objective To analyze the clinical effect of fumigation with Chinese medicine combined with rehabilitation training on hemiplegic limb spasm after stroke.Methods A total of 80 patients with hemiplegic limb spasm after stroke admitted to the Department of Traditional Chinese Medicine of Nanning Second People's Hospital from July 2021 to July 2023 were selected as the research objects,and were divided into observation group and control group by case number random grouping method,with 40 cases in each group.The control group was given traditional Chinese medicine fumigation,and the observation group was added rehabilitation training on the basis of the control group.Lower extremity blood flow velocity/lower extremity circumference,hemorheology index,incidence of deep vein throm‐bosis and lower extremity swelling,life ability score,Barthel score and Lovett grade were compared between the two groups.Results Lower extremity blood flow velocity(35.86±4.61)cm/s and lower extremity circumference(30.25±2.11)cm in the observation group were better than those in the control group(24.95±4.16)cm/s and(33.14±2.03)cm,the difference was statistically significant(t=10.994,4.086,P<0.05).Hemorheology index,incidence of deep vein thrombosis and lower limb swelling,Barthel score and Lovett grade in observation group were better than those in con‐trol group,the difference was statistically significant(P<0.05).Conclusion For patients with hemiplegic limb spasm after stroke,TCM fumigation and rehabilitation training can improve hemorheology,increase blood flow speed,effec tively prevent postoperative deep vein thrombosis,effectively improve life ability,and have high overall safety.
