腹腔镜子宫肌瘤剔除术和传统开腹术治疗子宫肌瘤的有效性及安全性分析

摘　　要：目的探讨子宫肌瘤应用腹腔镜子宫肌瘤剔除术方案与传统开腹术对比。方法选取2020年1月—2023年8月常州市肿瘤医院收治的80例子宫肌瘤患者为研究对象,按照随机数表法分组,每组40例。对照组运用传统开腹手术方案,观察组运用腹腔镜子宫肌瘤剔除术方案。对比手术指标、卵巢功能、炎性因子、疼痛应激、生活质量。结果观察组术中出血量为(63.22±3.91)mL,手术时间为(59.22±3.94)min,术后排气时间为(11.45±2.42)h,住院时间为(3.52±1.01)d,均少于对照组的(86.73±4.95)mL,(97.84±6.17)min,(29.33±3.24)h,(7.98±1.45)d,差异有统计学意义(t=23.572、33.365、27.963、15.963,P<0.001)。观察组卵巢功能、炎性因子、疼痛应激、生活质量指标均优于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论子宫肌瘤运用腹腔镜子宫肌瘤剔除术方案实施治疗,相较传统开腹术式,可提高手术实施质量,促进卵巢功能恢复,降低炎性反应程度,减轻疼痛应激水平,提升生活质量,确保临床安全性。Objective To investigate the application of laparoscopic myomectomy program for uterine fibroids com⁃pared with traditional open abdominal surgery.Methods A total of 80 patients with uterine fibroids admitted to Chang⁃zhou Cancer Hospital from January 2020 to August 2023 were selected as the research objects and divided into two groups according to the random number table method,with 40 cases in each group.The control group was treated with traditional open surgery,and the observation group was treated with laparoscopic myomectomy.The surgical indica⁃tors,ovarian function,inflammatory factors,pain stress and quality of life were compared.Results Intraoperative bleed⁃ing in the observation group was(63.22±3.91)mL,operative time was(59.22±3.94)min,postoperative exhaustion time was(11.45±2.42)h,and hospitalization time was(3.52±1.01)d,which were less than that in the control group(86.73±4.95)mL,(97.84±6.17)min,(29.33±3.24)h,and(7.98±1.45)d,and the differences were statistically significant(t=23.572,33.365,27.963,15.963,P<0.001).Ovarian function,inflammatory factors,pain stress,and quality of life indi⁃cators were better in the observation group than in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the dif⁃ference was statistically significant(P<0.05).Conclusion The treatment of uterine fibroids by laparoscopic myomec⁃tomy program can improve the quality of surgical implementation,promote the recovery of ovarian function,reduce the degree of inflammatory reaction,reduce the level of pain stress,improve the quality of life,and ensure the clinical safety compared with the traditional open abdominal surgery.
