变形杆菌属中抗菌药物亚胺培南稀释法和纸片法的对比研究

摘　　要：目的提高亚胺培南对变形杆菌属药敏报告的正确性。方法选取2021年5月—2023年8月广东省英德市人民医院临床标本分离出的208株变形杆菌属,对同一标本来源分离出的变形杆菌属同时进行抗菌药物亚胺培南仪器稀释法药物敏感试验和手工K-B纸片扩散法药物敏感试验,进一步作结果对比分析。结果仪器稀释法与手工K-B纸片扩散法试验结果呈耐药判读的相符率是52.63%,呈中介判读的相符率是64.71%,呈敏感判读的相符率是94.12%,从变形杆菌属中细分出来的奇异变形杆菌,普通变形杆菌及豪泽变形杆菌在研究试验中得出的相符率与总相符率保持一致,其耐药判读结果相符率为52.94%和50.00%,中介判读结果相符率为65.19%和61.11%,敏感判读结果相符率为92.86%和100.00%,试验数据可见相关统计表格。结论仪器稀释法检测抗菌药物亚胺培南药敏时,由于方法学的局限性,当其结果呈现耐药或中介时存在判读过高,可使用手工K-B纸片扩散法对药敏结果予以纠正,提高结果报告的准确性。Objective To improve the accuracy of the report of imipenem dilution sensitivity to proteus genus.Methods A total of 208 strains of proteus were selected from clinical specimens of Yingde People's Hospital of Guangdong Province during the period of May 2021 to August 2023.The drug sensitivity test of imipenem dilution method and manual Kirby-Bauer(K-B)method were carried out for proteus isolated from the same specimen,and the results were further compared and analyzed.Results The agreement rate between the instrument dilution method and the manual K-B method for drug resistance identification was 52.63%,the agreement rate for intermediate identification was 64.71%,and the agreement rate for sensitive identification was 94.12%.The coincidence rates of proteus mirabilis,proteus common and proteus Hauser subdivided from proteus genus were consistent with the total coincidence rates.The consistent rates of drug-resistant interpretation results were 52.94%and 50.00%,intermediate interpretation results were 65.19%and 61.11%,and sensitive interpretation results were 92.86%and 100.00%.The test data can be seen in the relevant statistical table.Conclusion Due to the limitations of the methodology,when the results showed drug resistance or intermediation,the interpretation was too high.Manual Kirby-Bauer method could be used to correct the drug sensitivity results and improve the accuracy of the result report.
