益肾活血汤治疗慢性肾脏病CKD 3~4期的疗效探讨

摘　　要：目的观察益肾活血汤治疗慢性肾脏病(chronic kidney disease,CKD)3~4期的疗效。方法选取2019年10月—2021年6月东台市中医院收治的86例CKD 3~4期患者作为研究对象,按照随机数表法分为对照组和观察组,每组43例。对照组采用常规治疗,观察组在常规治疗的基础上使用益肾活血汤治疗。对比两组治疗前后肾功能、中医证候积分和不良反应。结果治疗前,两组血肌酐、24 h尿蛋白以及尿素氮水平对比,差异无统计学意义(P>0.05);治疗后,两组血肌酐、24 h尿蛋白以及尿素氮水平均有下降,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组中医证候积分对比,差异无统计学意义(P>0.05);治疗后,观察组主症、次症积分分别为(9.76±1.85)分、(5.03±1.47)分,低于对照组的(11.99±2.33)分、(6.81±1.75)分,差异有统计学意义(t=4.915、5.107,P<0.05)。观察组不良反应发生率为11.63%,与对照组的18.60%比较,差异无统计学意义(P>0.05)。结论在慢性肾脏疾病CKD 3~4期患者的治疗中,在基础治疗的同时采用益肾活血汤,对于患者肾功能的提升有积极影响,不会增加不良反应的发生率,具有较高的应用价值。Objective To observe the efficacy of Yishen Huoxue decoction in treating chronic kidney disease(CKD)stage 3-4.Methods 86 patients with stage 3-4 CKD admitted to Dongtai Hospital of Traditional Chinese Medicine from October 2019 to June 2021 were selected as the study objects,and were divided into control group and observation group according to random number table method,with 43 cases in each group.The control group was treated with routine treatment,and the observation group was treated with Yishen Huoxue decoction on the basis of routine treatment.Renal function,TCM syndrome score and adverse reactions were compared between the two groups before and after treatment.Results Before treatment,there were no statistically significant difference in serum creatinine,24 h urinary protein and urea nitrogen levels between the two groups(P>0.05);after treatment,the levels of serum creatinine,24 h urinary protein and urea nitrogen in two groups were decreased,and the observation group was lower than the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in TCM syndrome scores between the two groups before treatment(P>0.05);after treatment,the scores of main disease and secondary disease in the observation group were(9.76±1.85)points and(5.03±1.47)points,respectively,which were lower than those in the control group(11.99±2.33)points and(6.81±1.75)points,and the differences were statistically significant(t=4.915,5.107,P<0.05).The incidence of adverse reactions in the observation group was 11.63%,compared with 18.60%in the control group,the difference was not statistically significant(P>0.05).Conclusion In the treatment of patients with chronic kidney disease CKD stage 3-4,Yishenhuoxue decoction combined with basic treatment has a positive effect on the improvement of patients'kidney function,without increasing the incidence of adverse reactions,and has high application value.
