探讨慢性硬膜下血肿钻孔引流术后复发的危险因素

摘　　要：目的探究慢性硬膜下血肿(chronic subdural hematoma,CSDH)钻孔引流术后复发的危险因素。方法选取2015年6月—2022年12月江苏省句容市人民医院接受钻孔引流术的CSDH患者中纳入107例作为研究对象,对术后复发情况展开回顾性分析,根据调查结果将纳入患者分为对照组(未复发,n=84)、研究组(复发,n=23),根据基础资料调查问卷等调查结果展开单因素分析、多因素Logistic回归分析。结果Logistic回归分析结果显示,年龄、血肿体积、血肿厚度、术前中线移位均为CSDH患者钻孔引流术后复发的独立危险因素(OR=1.054、1.891、1.431、1.721,P<0.05)。结论CSDH患者钻孔引流术后复发的独立危险因素包括年龄、血肿体积、血肿厚度、术前中线移位,可将上述因素作为评估患者是否存在复发因素的独立指标。Objective To explore the risk factors of recurrence after drilling and drainage of chronic subdural hematoma(CSDH).Methods 107 patients with CSDH who underwent drilling and drainage in Jurong People's Hospital of Jiangsu Province from June 2015 to December 2022 were included as study subjects,and a retrospective investigation of postoperative recurrence was conducted.According to the results of the survey,the included patients were divided into the control group(no recurrence,n=84)and the study group(recurrence,n=23),and unifactorial analysis and multifactorial Logistic analysis were carried out according to the results of the survey such as the basic information questionnaire.Results Logistic regression analysis showed that age,hematoma volume,hematoma thickness,and preoperative midline shift were all independent risk factors for recurrence after drilling and drainage in CSDH patients(OR=1.054,1.891,1.431,1.721,P<0.05).Conclusion The independent risk factors for recurrence after drilling and drainage in patients with CSDH include age,hematoma volume,hematoma thickness,and preoperative midline shift,and the above factors can be used as an independent indicator to assess whether there are recurrence factors in patients.
