摘 要:目的评估利用呼吸机与肺表面活性物质结合应用于新生儿呼吸窘迫综合征患者的临床效果,以及对患者血气指标的影响。方法收集洛阳市妇幼保健院于2020年12月—2022年12月收治的90例新生儿呼吸窘迫综合征作为研究对象,采用数字表法随机分为对照组(n=45)和研究组(n=45),对照组采用常规治疗,研究组在对照组的基础上利用肺表面活性物质结合呼吸机支持治疗。对比两组患儿治疗后的氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、pH值、呼吸机有效指数(VEI)、呼吸指数(RI)及氧合指数(OI)、呼吸机使用时间及住院天数。结果治疗1d后,研究组的PaO_(2)、pH显著高于对照组,PaCO_(2)显著低于对照组,差异具有统计学意义(P<0.05);研究组的VEI显著高于对照组,RI、OI、呼吸机使用时间及住院天数显著低于对照组,差异具有统计学意义(P<0.05)。结论在新生儿呼吸窘迫综合征的治疗中,肺表面活性物质结合呼吸机支持治疗可显著改善呼吸窘迫综合症状和氧合功能,这一综合治疗策略在临床实践中值得推广应用,以提高新生儿呼吸窘迫综合征患者的治疗效果。Objective To evaluate the clinical effect of using ventilator in combination with pulmonary surfactant in patients with neonatal respiratory distress syndrome and its effect on blood gas indexes.Methods 90 cases of neonatal respiratory distress syndrome admitted to Luoyang Health Center for Women and Children from December 2020 to December 2022 were collected as research subjects,and randomly divided into control group(n=45)and research group(n=45)by numerical table method,the control group was treated with conventional treatment,and the study group was treated with pulmonary surfactant combined with ventilator support on the basis of the control group.The partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2)),pH,ventilator effective index(VEI),respiratory index(RI)and oxygenation index(OI),duration of ventilator use and length of hospital stay were compared between the two groups.Results After 1 day of treatment,PaO_(2) and pH of the study group were significantly higher than those in the control group,and PaCO_(2) was significantly lower than that in the control group,with a statistically significant difference(P<0.05),the VEI in the study group was significantly higher than that in the control group,and the RI,OI,ventilator duration and hospital stay were significantly lower than those in the control group(P<0.05).Conclusion Pulmonary surfactant combined with ventilator support therapy can significantly improve respiratory distress syndrome and oxygenation function in the treatment of neonatal respiratory distress syndrome,and this comprehensive treatment strategy is worth promoting in clinical practice to improve the treatment effect of patients with neonatal respiratory distress syndrome.
