不同雷公藤制剂联合紫癜Ⅰ号方治疗儿童反复发作性过敏性紫癜血热妄行证的临床疗效及安全性观察

摘　　要：目的:观察不同雷公藤制剂(雷公藤颗粒、昆仙胶囊)联合紫癜Ⅰ号方治疗反复发作性过敏性紫癜血热妄行证患儿的疗效及安全性。方法:将符合纳入标准的108例反复发作性过敏性紫癜血热妄行证患儿随机分为试验1组、试验2组和西医对照组,试验1组给予0.025g·kg^(-1)·d^(-1)雷公藤颗粒(相当于饮片0.5g·kg^(-1)·d^(-1))+紫癜Ⅰ号方,试验2组给予0.3g·10kg^(-1)·d^(-1)昆仙胶囊+紫癜Ⅰ号方,西医对照组给予氯雷他定+维生素C+泼尼松片,治疗4周,评价皮疹疗效、临床疗效,检测0、2、4周末血常规、尿常规、肝肾功(血热妄行证)以及0、4周免疫六项(IgA、IgG、IgM、IgE、C3、C4)。结果:试验1组、试验2组皮疹总有效率均为93.33%(28/30),临床总有效率均为96.67%(29/30),均显著优于西医对照组[80.00%(24/30)、86.67%(26/30)](P<0.05)。试验1、2组总不良反应发生率均显著高于西医对照组(P<0.05)。试验2组与西医对照组比较治疗后IgA显著下降(P<0.05)。3组肾脏损伤的发生率比较,差异无统计学意义。结论:雷公藤颗粒、昆仙胶囊联合紫癜Ⅰ号方治疗儿童反复发作性过敏性紫癜血热妄行证具有显著疗效,二者疗效无显著性差异且雷公藤颗粒有更低的不良反应发生率。Objective:Different Tripterygium preparations(Tripterygium wilfordi Granules,Kunxian Capsules)combined with Purpura No.Ⅰ Prescription were used to intervene in children with recurrent Henoch-Schonlein purpura with blood heat delusional syndrome.Methods:A total of 108 children with recurrent Henoch-Schonlein purpura who met the inclusion criteria were randomly divided into test group 1,test group 2 and western medicine control group.Test group Ⅰ was given 0.025 g·kg^(-1)·d^(-1)Tripterygium wifordi Granules(equivalent to 0.5 g·kg^(-1)·d^(-1))Tripterygium wilfordii Decoction)+Purpura No.Ⅰ Prescription,test group 2 was given 0.3 g·10kg^(-1)·d^(-1)Kunxian Capsule+Purpura No.Ⅰ Prescription,and the western medicine control group was given loratadine+Vitamin C+Prednisone Tablets,treated for 4 weeks,evaluated the efficacy of rash and clinical efficacy,detected blood routine,urine routine,liver and kidney function at O,2,4 weeks,six immune indexes(IgA,IgG,IgM,IgE,C3,C4)at O weeks and 4 weeks.Results:The total effective rate of skin rash in test group I and test group 2 was 93.33%(28/30),and the total clinical effective rate of them was 96.67%(29/30),which was significantly better than the western medicine control group[80.00%(24/30),86.67%(26/30)](P<0.05).The total adverse reaction rates of test group 1 and test group 2 were significantly higher than those of the western medicine control group(P<0.05).IgA in test group 2 decreased compared with the western medicine control group after treatment(P<0.05).The incidence rates of kidney injury in three groups had no significant difference.Conclusion:Tripterygium wilfordi Granules,Kunxian Capsules combined with Purpura No.Ⅰ Prescription had significant curative effect on the treatment of children with recurrent Henoch-Schonlein purpura with blood heat delusional syndrome,and there is no significant difference in the curative effect between the two,and Tripterygium wilfordi Granules has a lower incidence of adverse reactions.
